Effexor xr 20 Years on now causing problems

Hello, I am new here and really need some sound advice. I was diagnosed with GAD and Severe depression twenty years ago. I was prescribed 75mg of Effexor ER. At first it worked well. Several years went by and I began feeling anxious and depressed again. My GP recommended to increase the dosage to 150mg. I was to take 75mg twice daily. This worked and I felt much better. Four years later and I began to feel anxious and depressed again. Once again, my GP increased my dosage. 225mg per day. 150mg in the morning and 75mg in the evening. Yes, it worked and life was somewhat normal. Jumping forward Several more years and...same thing. I was prescribed Prozac to take along with the Effexor. I cannot recall the dosage of Prozac. I tried this for a year and noticed no changes. Now, my GP adds in XANAX. One mg per day. Xanax seemed to be the only thing that helped. I decided to stop taking Prozac. No negative changes. I thought I would cut my Effexor dosage back to 150mg. Of course, as expected, withdrawal effects. Muscled through that for a month. It was HELL! I waited four months and began weaning off Effexor. I dropped my dosage to 75mg daily. I was still taking Xanax to help with the withdrawal. I tried to wean off 75mg but it was just too much. Jump ahead several years later to the present. I have been at this dosage the whole time, not skipping or changing the dosage time. I am having panic attacks, brain zaps and severe depression. I can physically, hardly function. I know that common since dictates changing doctors. I ended up at The Mayo Clinic in Jacksonville. They decided to add welbutrin into the mix.. After twelve months with Mayo and multiple requests to change the cocktail, no luck.. They recommended to keep giving it a chance. I am at a breaking point. I am 59 years old, male, physically very fit but I have high BP, Congestive heart failure and CKD. I know that sounds contridictive but I work out. Lately, I started researching my situation and found this website. I really need help. Please, if you have any suggestions, I am begging you to share. Please
