I just checked my diabetes results on diabetes my way and seen my eGFR was 36 ml/min should I be worried down from 54 last check up

    Noon,

    What is your creatinine? Also, if you don't mind, how old are you? And finally, when was your last lab test run, i.e., when your eGFR was 54? Also, how does your A1C typically run?

    If you have 2-3 eGFR readings close to 36 your kidneys would be functioning in the lower half of Stage IIIB. An eGFR of 56 is the upper end of Stage IIIA. So depending on how close together your labs are run, this could be concerning.

    Are they going to run a follow-up lane to confirm these recent results? Do you currently see a nephrologist (kidney specialist)? If not, I'd strongly suggest that you ask for a referral to a nephrologist as soon as possible.

    I have type 2 diabetes and chronic kidney disease so please feel free to send me s private message if you'd like to visit more. I'm not a doctor but I'm certainly willing to share anything I can with you. Fortunately my A1C is totally stable in normal range with no medication. Nevertheless, my renal function has still deteriorated.

      to give you some background info about 2 month,s ago I had severe pain in my right kidney went to doc gave me antibiotics and refered me for a ct scan ,got this about 3 weeks ago went to see doctors today one for result of ct scan and one for diabetes review .The doc said there was a stone in my left kidney but would just leave it as I was not in pain could not explain about right kidney (still get pain in it but not as sever) case closed.Diabetes doctor said my diabets was great no problems with it and said he would stop my sitagliptin,gliclazide oxybutynin and quinnine so down to two metaformin a day.Forgot to say doc would not let me see ct scan or print it out

      Noon, definitely good news on your diabetes management!! You and I are close to the same age. My renal panel data looked fine, i.e., in the normal range until I had s severe gall bladder attack in June of 2011. I couldn't keep any fluid or good down for about 4 weeks as they did s variety of tests. Naturally I became severely dehydrated during that time. By blood pressure plummeted. Once my hall nladder was removed we discovered that my renal function had deteriorated significantly. My kidneys have plummeted 2 more time since. Each time was s major decrease in renal function. Each time was preceded by prolonged dehydration due to illness. My kidneys then rebound some before stabilizing for 3 years or do--until I manage to become quite ill again.

      So o try very hard not to get sick or get UTIs. I also have to work to stay hydrated. My nephrologist seems to think that with my A1C in check there shouldn't be major deterioration related to the diabetes. I certainly hope he is right.

      How is your BP?

      Glad to hear you're under regular medical care for all of this. As I've already said; I'd definitely get in for additional appointments with my nephrologist if my data did what yours has done. It may be just s normal gluctuation or some atypical data for you got done readob--but best to know for dure🐶

      No you, this data goes look a lot like mine used to look before my kidneys deteriorated to Stage IV.

      My primary care doctor wasn't too worried when my data was in this range, but as I shared earlier, it can plummet into low Stage IV or Stage V very quickly. So if I had it to go over again I would have insisted to be referred in to a reputable nephrologist while my kidneys were working as well as yours are now. Once they plummet it's very rare to recoup the functionnthst has been lost.

    Well it certainly needs to be discussed with your doctor.  The can be fluctuations in eGFR from one blood test to another  due to lots of reasons.  But I agree that your change is significantly more than just moving up or down 5 or points.  When is your doctors appointment?  The doctor will have access to a lot of other results and be able to identify why the drop.  A level of 36 certainly classifies you as stage 3 CKD (Same as me).  I would imagine they will do another test to see if this is just a one time unexplainable event, or to determine more accurately why the drop.  Let us know.  One thing everyone here advises each other is do not panic.  Go to the doctor, sooner the better so that you don't drive yourself crazy with worry, and see what it is all about.

      Noony, is there another doctor you can see in the Practice?  I have CKD Stage 3b. That is a steep drop in under a year.  The least they should do is repeat the blood test in the next few weeks to see if it is either stable at that reading or whether it may have increased back up.  I know a lot of doctors won't refer patients to kidney consultants because basically the consultants themselves aren't particularly interested if patients are within Stage 3 of kidney disease.  Very many patients, particularly those over a certain age are diagnosed within Stage 3 and this is not considered to be a problem in that age group as long as the reading remains stable and is not on a continuing downward decline.  Have you taken any medication in the NSAID range, such as Ibuprofen?  Or have you had a UTI recently needing antibiotics - both can knock kidney function.  One AB I have taken for UTI reduces my eGFR by 10% but luckily the level recovers (or has up to now) with a repeat blood test after 2-3 weeks.  Perhaps take a urine specimen to the surgery and ask them to dip it for any sign of a 'silent' infection just for reassurance.  The eGFR is worked out from the creatinine reading so it is always a given that if the creatinine is raised the eGFR will have lowered.  The two go hand-in-hand.  Be strong and stand up for your rights - we, the patients, pay the doctors' salaries!  If you can't get any joy with any doctor in the Practice then ask to see the Practice Manager.  Good luck on all fronts!
