How is your blood pressure? During masturbastion you are straining and excited, it's like a workout. Does your head feel this way when you do any kind of other strenuous activity? I think I would see a general practitioner for a good physical to see if there is anything else going on with you like your blood pressure and etc. As for not all the fluid coming out, there is something that some mean have that is called retrograde ejaculation. It's where some or all the fluid goes into the bladder instead out of the penis during an orgasm.  Retrograde ejaculation is not harmful and I do not believe that would be the cause of your problem.
