embolization fibroids for large fibroid

Hello, wow I have been searching the internet for other alternatives and read a little info about UFE. I have had fibroids for about 5 years well maybe more but was seen by a dr around that time. Since then I have had heavy bleeding and about 3 years ago had an IUD for the bleeding. It never really worked and I just continued with the inconvenience of bleeding.  I was never told how rapidly they would grow and about a month in a half ago. I bled so bad and had severe clots I thought I was dying.  I about passed out and felt body drifting very scary. I finally went into emergency and they did blood work. you are supposed to be at 14 to 15 for blood and I was at 4.  If I wouldn't have went in lets just say would be posting this.  I had to be admitted and had 4 pints of blood for the transfusion.  They did my MRI and test I have one fibroid that enlarged as if I'm 5 months pregnant and over 100 small m&m size ones growing.  I was only given options for abdominal hysterectomy and I am devastated.  I have had my kids and I am over 40 so they acted like no big deal to just get it done.  I have been so sad and this has taken over my life with worry.  I am supposed to go get another opinion to day to see if I'm a candidate for UFE. I know there are other women going through this I posted in replies to other stories.  I am so confused but I am feeling pushed into a hysterectomy by doctor and by some people that know of this and say to me no big deal you have had children.  I just want to try other alternatives. My concern is the larger fibroid it scares me and I'm afraid this wont work but I am willing to try.  If anyone had great results for big fibroids and a lot of other ones please let me know of the experiences you went through and the outcome.

Hi Kristen. I also was losing blood clots so big I thought at one point I was hemorrhaging and it took several years for one Dr to sit up and take notice of the impact of this on my well being and general health. I am pushing for partial hysterectomy as this is my best option for me but I totally feel for you. I found this site only yesterday and already have found it a godsend. Keep your spirits up and know that there is always someone ready to listen.

      Hi Tracey.

Good luck to you with the surgery. I have been going back and forth so much I almost did go through with hysterectomy. I support myself and although I have insurance the company has no pto so it's tough. Thanks and good luck. Would like to hear more of your recovery. Hope your bleeding has slowed down.

Hi Kristen. At the moment I'm on tranexamic acid tablets as and when needed and have also had the mirena coil fitted and although I am bleeding and cramping still virtually on a daily basis it's bearable now. My next consultation is Mar 12th with surgery hopefully soon after. I will let you know recovery details

      Yes Tracey

Ease let me know all of the things we are dealing we need to just breath and remember we will get through this. I just came from radiologist Dr he was awsome. I have to wait for I surance approval then have mobilization done as early as mid Feb the large fibroid is by muscle so it does help with the location he stated. My thought and prayers within the coming weeks for u.

My advice is too wait to see if you are a candidate. The NHS has great doctors you may need to get a second opinion. I suggest you do your resesrch on the different type of procedures available

    Hi Kristen,

I am in the same position with deciding on the hysterectomy. Although I get that it's a routine procedure to the doctors and doesn't seem to be a big deal unless you want future kids, I have been trying for 5 years to avoid it. I've taken Lupron like meds to try to shrink it, etc. It is not pushing against a kidney, so more pressure to have the surgery.

Anyway, to your question. I had a consult for UFE for this very large fibroid. At first the physician said he didn't think it would work well on a large fibroid. Then at a later date, he sent me a paper that said there was success with larger fibroids. I decided against that procedure. I am currently investigating global fibroid ablation, a day surgery procedure.

Here is the title of the paper - and I will try to paste in abstract too. Best wishes on your decision.

"Is Uterine Artery Embolization for Patients with Large Myomas Safe and Effective? A Retrospective Comparative Study in 323 Patients"

Hye Jeong Choi, MD, Gyeong Sik Jeon, MD, Man Deuk Kim, MD, Jong Tae Lee, MD, and Jung Hyun Yoon, MD

Abstract:   Purpose: To evaluate the effectiveness, safety, and complications of uterine artery embolization (UAE) in women with large fibroid

    tumors.

Materials and Methods: From January 2005 to February 2011, 323 patients underwent UAE for symptomatic uterine leiomyomas without adenomyosis and were included in this study. Patients were divided into two groups: those with a large tumor burden (group 1; n 1/4 63), defined as a dominant tumor with a longest axis of at least 10 cm or a uterine volume of at least 700 cm3; and the control group (group 2; n 1/4 260). Tumor infarction and volume reduction were calculated based on magnetic resonance imaging findings. Symptom status was assessed with a visual analog scale. Postprocedure complications and repeat interventions were recorded. The data were analyzed with appropriate statistical tests.

Results: No significant differences were seen between the two groups in volume reduction of dominant tumors (46.5% in group 1 vs 52.0% in group 2; P 1/4 .082) or percentage volume reduction of the uterus (40.7% in group 1 vs 36.3% in group 2; P 1/4 .114). Also, no significant differences were seen between the two groups regarding satisfaction scores at immediate or midterm follow-up (P 1/4 .524 and P 1/4 .497) or in the presence of procedure-related complications (P 1/4 .193).

Conclusions: UAE outcomes in large fibroid tumors were comparable to those in smaller tumors, without an increased risk of significant complications. Tumor size may not be a key factor in predicting successful outcomes of UAE.

      Hello Jamie

I am sorry to hear about it. I know the stress. Since the place contacted you for embolization what made you decline? I think I looked into ablation I thought if correct read if they were bigger they couldn't do the procedure. I may be wrong but I'm sitting here waiting to be seen be radiologist for consultation. I will keep you updated. This is even scary just waiting for that

    Hi Kristen

You should try UFE it is not a bad procedure and it shrunk my Fibroid from 11 to 7.5cm this made me eligible for a Laprascopic Myomectomy much less invasive. I got diagnosed in June 2014. I have never regretted having UFE but I had to stand my ground to get it.dont let them push you to Hysterectomy it’s your body your decision.

    Good luck

      Thank you Patricia

I just gpt back from the radiologist Dr. He was amazing and he is going to have my insurance sent through so that they can get an ok. He is planning on having my procedure done as soon as 2 weeks. The big one is next to muscle so he says makes it easier well less complicated to pass. I am happy yet scared though.
