Epididymal cysts

Hi, I get pain and dull heavy aching at the top of my right testicle, I've been told I have a small less than 2cm epididymal cysts on the head of the epididymis.

Urology have said they will not remove the cysts unless it gets bigger than 2cm, I find this very unfair as the pain I get is excruciating. They say they cant be sure that removing the cysts will stop the pain, they say it may make things worst.

Anyone else in the same boat as me?

You bet I am in the same boat. I just posted asking if anyone has had their testicle removed because of what you describe. I have the same problem.

My urologist said it is not practical to remove the cyst. Mine is almost an inch across and is ruining my life. My urologist will perform what is called a scrotal nerve block in 2 weeks. It is very easy - it takes one minute where he just injects a nerve blocking agent into the epididymis. He said it should block any pain for a couple of months and the procedure can be repeated as often as needed. It is just cortisone. But he also said that if it doesn't work he will remove the testicle which he said is no big deal as the other testicle can take over all the work. I don't know about that which is why I put out my post an hour ago. Anyway ask your doctor about the nerve block first. The epididymis can also be removed but that is a tricky surgery. Good luck.

Hi, I was offered the nerve block last month, but I turned it down in fear of what the specialist said to me. He told me the nerve block injection may not work, it may make things worst, he also said about some side effects, then tells me I could end up with infection, he basically talked me out of it.

I ask them if they would just remove my right testicle, they said no. They explained removal may not stop the pain, could make things worst they said.

I'm going to ask them again in January to remove it, they need to do something that is worth while.

Let me know how you get on, good luck buddy.

I know what you mean about ruining ones life, my pain controls my life completely, everything I do, its so upsetting.

I really would insist on the nerve block as a first try. I know a lot of guys who have had it and many were helped. The ones that it didn't help had no ill side effects. If you do get an infection there are good antibiotics to get you over it so that is no big deal. My nerve block is on Jan 15 and I am looking forward to it with some hope. Everyone reacts differently to things and it just may be the ticket for us. Good luck.

I'll post a new discussion and ask if anyone will share their experience of a nerve block.

Thanks Howard, I may just do that, I should have went for the nerve block injection really. I'm due to go back to the urologist on 11th, I will see what they say.

I've read that some men have reported the nerve block injection totally gets rid of the pain, pain hasn't come back. But then I read case's where the never block hasn't worked, its got to be worth a try I guest.

I haven't found any posts on here where the nerve block injection has worked, I wonder why that is?

I cannot find anything as well. I will make an appointment with my uro to discuss some of these concerns about the nerve block injection before I actually have it.

Another thing to consider too is that out symptoms are sometimes caused by a varicocele which can be fixed by an interventional radiologist or a urologist. I know I have a varicocele from a scrotal ultrasound I had last year. That might be worth a first go at fixing the problem. Ask your doctor about that too.

Yes, I have had 5 ultrasound scans over the past 12 years, and each scan has shown varicocele's and cysts on both testicles.

I don't get any pain on the left testicle, pain is always on the right one, the right one has a cysts on the head of the epididymiss, but the urologist still says he doesn't want to remove the cysts or varicocele's.

They always send me to the pain management clinic, this is when they said they would try me with a nerve block injection, but I got scared and changed my mind.

When I go back in January I will have to get them to do something, maybe even the nerve block. My general doctor tells me not to go under the knife, he tell's me if I can keep managing the pain with pain killers and loose clothing, keep doing that, he reckons operations don't always work, can make things worst.

Its such an awful position to be in isn't it?

It sure is. My urologist sent me to another urologist who specializes in testicular pain. He gave me the following 5 options in order of increasing undesireability: take ibuprofen on a regular basis; take nerve pain killers like Lyrica; get a scrotal nerve block injection; get the epididimys removed; get the testicle removed.

I would never take drugs like Lyrica so my next option is the scrotal nerve block which I will have on Jan. 15. To avoid infections from the procedure I will take an antibiotic, Keflex, for a week before the procedure. If I were you I would give it a go since if it works it can be done every few months forever. Also you should look into getting your varicoceles fixed by an interventional radiologist and not a urologist. The IRs have very simple non-surgical procedures that are quick and easy and very effective. Merry Christmas! Too bad no sex as it would send me into another flareup!

Get a testicular ultrasound to check for a varicocele on both sides. Your symptoms are consistent with varicoceles and can be easily fixed.
