HI,
Yes just send in everything you have. With ESA they don't look at any evidence you sent in previously. Each time a person is reassessed is like a new claim. Good luck when the time comes.

You could write a diary for a week or 2, ask someone that knows you well to write a letter for you, printed list from GP of all current medication, future applointment letters aren't classed as evidence for PIP or ESA. You could ask your GP if he/she has any other medical evidence from your records that you can use. There will most likely be a charge for this though. Is this for PIP or ESA?

Just send any evidence you have regardless of how old it is, they'll either use it or not. The only thing that's not classed as evidence is photos and furture appointment dates. My daughter had a PIP home assessment this week and the first thing he said to me was " i've read the evidence you sent it" so they do sometimes read it

Ok so this has to be for your DLA to PIP transfer correct? Have you started off you PIP claim yet? If you haven't, when you do then you can ask for your DLA file (evidence) to be taken into consideration.
