    Hi,

It's age UK not age concern and these are a charity that help older people and won't be able to help you with a benefit for working age people. Contact either your local CAB or Welfare Rights and they will hopefully be able to help you, if you can get an appointment in time. Bare in mind that the form needs to arrive with the Tribunal within 28 days of the MR decision being made. You also need to make sure you send the MR decision letter with the form otherwise the Tribunal will refuse your request.

    Good luck.

      Hi,

You'll go back onto assessment rate i'm afraid because you've been found fit for work. Fit/sick notes will be needed and if you haven't been claiming another benefit while you wait for the Tribunal request then if you need your ESA to be backdated to the date it stopped your fit/sick note will need to be backdated to the date you want your ESA backdated to. Couples rate is £114.85 per week.
