Hi,
They are contacting everyone that transferred from IB to ESA. As no one on an internet forum can tell you whether you should have been claiming an Income related top up then yes, it's worth filling in the form in and returning by the date stated in the letter.
To assess you for the income related top up then they look at past income from living with a partner (income from parents isn't counted) savings/capital of £6,000 will affect any top you can claim, anything over £16,000 then you won't be entitled to any back dated money. Income from pensions etc.
It can take 12 weeks to receive a decision on this. If they owe you any money it will be back dated to the date in the letter.

As far as I am aware, then no it has nothing to do with reassessment. You would get an ESA50 form if you were going to be having a reassessment.

The Enhanced disability premium is £16.40 per week from April this year, less in previous years. I have no idea how much you'd could be entitled to i'm sorry.
It's not a re-assessment like you have when you fill out the ESA50 form. It's to be assessed for the Income related top up.

It's not the same year for everyone. Some people transferred from Incapacity benefit as early as 2011 and others as late as 2014. They are backdating money to November 2011. Some people are being paid as much as £20,000.
