Esophagectomy or Gastrotomy for Achalasia

I would really just like responses just from those who have Achalasia, type 1, and have had a Heller Myotomy & Fundoplication, and balloon dilations. At what point did either an Esophagectomy or a Gastrotomy [tube feeding through the abdomen] become a necessity?

    I think that the criterion for an oesophagectomy being necessary would be that the oesophagus had become big and baggy, creating malnutrition, and not likely to be responsive to other surgical solutions; and/or food starting to aspirate into the lungs.

    ​The feeding tube would be the means of providing nutrition that was not possible by normal methods.

      Thanks Alan.  Either of those two treatments seems to be the road I'm travelling on.  No malnutrition at this point, which is good.  I'm very careful to get nutrients into this body.
