Failed Urolift.

I had the Urolift done in August 2018 to "hopefully" cure my recurring UTIs. 2 months post surgery and "boom" a UTI. Not only did I NOT stop taking Rapaflo but he added Finasteride. My symptoms of BPH seem to be a little better at times but I still get up 2-3 times at night. I had 2 Uro's tell me my problem was BPH but I'm just not convinced (now more than ever). My normal retention was around 30 ml and post surgery was 50 ml. I'm doing a follow up in Jan and he has already began discussing the "T" word. I told him that I wouldn't discuss it because I didn't believe it was BPh that was my problem. He said there were a few tests to be done to rule all else out. Could use some advice here as to what I should be pushing him to do next. Thanks....Billy

Hey there Billy
Just because you get a UTI does not mean that the Urolift failed it may take longer for your bladder to relax I had the Urolift going on 4 years and I still get kidney and bladder infection.
My prostate is fine still wide open. I had 4 clips put in and they are in the same place that he put them
Don't let them try to talk to you about the T word. I was told by my doctor that most doctors do all the test if they do not fine anything they will assume that it is the prostate. But what happens if you have the prostate cut out and you still have the problem. They will not put it back. Oh well.
Please give it some time. How many clips did he put in. Maybe you need more clips they can go up to 8.
Good luck.....Ken

These Urolift failure stories make me wonder how much training these Uros have with the procedure. My (young) guy had two representatives MD's in the surgery room even though he had at least 35 procedures under his belt ( I specifically asked him how many.) The results have been exactly as expected so I may have just lucked out.
Maybe you need to have a second opinion. I know it is uncomfortable to ask for that but these days you must be very proactive in your healthcare.
What is the "T" word?

You know RESUM is not contraindicated after a Urolift or as Ken says, maybe more clips. Keep pushing for the least invasive procedure. If a TRUP is all they will offer I suggest you fire them. You are in control of your healthcare.
Also, 30 ml retention or even 50) is not bad. I believe up to 100ml is normal. Your URIs are probably not due to retention. Perhaps you need to have a comprehensive blood panel done to see why your body is not dealing with infections as it should.
How old are you Billy? What other symptoms do you have? And BTW, 2 times up at night is sort of not so bad. A bother but not that abnormal.

Hi Billy,
The only way to know if the prostate is out of the way is a cystoscopy. Send a camera up the urethra and look for a narrowing all the way into the bladder. When the camera is in the bladder look at the bladder neck and see if it's a funnel shape pointing down toward the prostate. It should be. What sometimes happens is the prostate can grow up into the bladder and push the bladder neck up into a volcano shape pointing up. It wasn't designed to work in that orientation. That's my situation. I've had 2 PAE's and gotten some relief because the path through the prostate is open, but the bladder neck is still pushed up. So I am still on Flomax and did the 8 months of finasteride which made no difference. I know this can be frustrating but doctors don't know everything, they just know what they know. They can't tell us why it's worse at night for example. Keep troubleshooting, Verify where the blockage is if you can and then try to deal with that.
Good luck,
Bruce
