Fissure and haemroids after haemroidoctomy

Hi. Last year in September I had a haemroidoctomy and an anal stretch for a fissure. Recovery was tough but I thought it was worth it until last week where I started with pain again. I felt I never recovered 100% so I went to the doctors a few weeks back before lockdown and the doctors said they could see haemrhoids but when I saw my specialist she said they are blood blisters and they can be very painful. But the pain subsided until now. I have had banding done and botox injections 7 times over the years. To me it feels like a fissure pain but I have lumps but can't get seen yet. I have been given rectogesic cream, but no applicator to reach my fissure and it gives the worst headaches! But I will continue as people swear by it.

So just looking for advice, it is likely a fissure will heal up on it's own? Any cream people can recommend? I have been having a few sitz baths a day that ease it a little bit.
