Fissure and Piles - Can't get to doctor due to duties remote area

Hello All,

Have gone through almost all threads in this discussion and need your device.

I am 29 and feel pain while passing bowels. The pain is like passing a needle but lasts for 2-3 second. The biggest problem comes afterwards; I feel extreme burning sensation and throbbing that continue for 4-5 hours immediately after BM. I have been facing this for the past 2 months.

I could not get it checked it with a specialist as I am on corona related duties in remote area. I consulted with my doctor friend and he advised me GNT ointment. I am using it for past 5-6 days (2 times a day). While applying cream, I do feel bump/lump outside anus. I think it's fissure + piles.

I have BM/constipation related problems for the past 5-6 years and felt pain on and off. Never was comfortable with passing stools during that time but the feeling these days is extremely irritating. I have changed my diet too since this happened 2 months back. For now, the sharp pain while passing stool is manageable but afterwards burning sensation/discomfort is unbearable and is taking psychological toll.

Please help.

    Hi there ,

    I've had problems with fissures on and off for just a year now my last reoccurance was about a month ago and I have never experienced the pain like that before I'd rather give child birth the burn was the thing that got me and your right the impact it has on you mentally is as bad as the pain . My doctor has put me on rectogesic for the next month but the one thing I find that helps with the burn is sudo cream like nappy rash cream i use for my kids it takes away the burn straight away i really do hope this helps . The best of luck with it i wouldn't wish this pain on my worst enemies .

    I confirm what Karen said. keep on using the ointment. it will work and do miracles in the near future (my experience) and use

    sudocream at least 15 minutes before going toilet. and put it like 5 times at day for the first few weeks. yes it will look like a mess. also I thought I had hemorrhoids too but it turned out its a fistulae. what a joy. trying to find a private surgeons as sick and tired of living like this. i understand cancer patients needs priority but there are other people that are in pain too

      For how long should I keep on using GNT? I have been facing the problem for the past 2 months but have started using ointment for the past 5-6 days. I am assuming I have chronic fissure.

      Don't have time for undergoing surgery.

    Thanks a lot for your response. I have been using GNT for past 6 days (2 times daily) but so far, there has not been much help. At the moment, I have bowel movement every alternate day (not daily). The condition has taken psychological toll, particularly after BM. I dread going to toilet now and need to prepare myself mentally 3-4 hours earlier. Can't have it more now. Enough!

      this is my third month. it started working on me after 5 weeks. now after a bowel movement I don't get the pain afterwards nor the burning. but the fissure is still there and need botox ASAP. I read you're not supposed to use the ointment for more them 2 months, but my consultant and my gp told me to keep on using it as its keeping it under control. on the other hand I'm fightin with the fistula. after bm they get really swollen and painful

      Thanks for your response. Does it mean I need to bear this pain for more 4 weeks as I have just started using for the past 1 week? I really can't go on for that long.

      I am not sure how should I explain unease after BM which continues for 4-5 hours. It is burning sensation + throbbing. Not pain as such but a feeling which prevents you from doing anything.

      this is about my experience with it. and how much time my body took to started the process of healing. not fully tho. just to make it bearable . so I started like you with the burning . after few weeks went to like having sharp glass every bm . few weeks after bm that's the worse bit. excruciating pain and muscle spasm that last up to 6/7 hours. that's when I started using the cream. middle of couid, doctors not available and started doing research. then gradually goes better . drink and I mean drink lots and lots of water. eat less and try some fasting to speed up the healing process. what I just said doesn't have to happen to you. this is my experience

      yes. I am having very less proper meal. Only fruits, oats, milk etc. Only 1 time meal daily.

      When I put GNT down there, I feel lump as well. Will it be external haemorrhoid?

      honestly i don't know. I went to the nhs hospital as I was in pain and the stupid moron doctor told me I had skin tags and gave me a paracetamol to take care of it. then I spent 200 quid for a private consultant and he said they were fistulas . even the consultant called that doctor a useless moron. if you can go private or to your local hospital and get yourself check out. DO NOT waste time

    Thank you. Tried to get an appointment but could not and now have to wait for another week.

    The update is that I have now been using GTN+stool softener for the past 3 weeks. The stools have become soft and the pain while passing stools has reduced a bit but there is extreme burning sensation+tightness+extreme discomfort (not pain as such) AFTER bowel movement. I am thinking of taking Ibuprofen after next bowel movement (which is tomorrow and I am extremely scared of post-bm burning and discomfort which continues for 5-6 hours and I can not sleep with that sensation. My bm is usually at 10 PM every alternate day).

    Not sure what to do, thinking of consulting some doctor through zoom/skype etc.
