You have a bad case of phimosis. I know people don't like to hear it, but you'd benefit a lot from circumcision. You can always try a steroid cream first (like betamethasone) but it can take 1+ month to work and it's possible it may not improve by much. Try this conservative method first. Good luck.

When did you first notice the redness, how long ago?
What age are you?
Would you consider getting circumcised or not?
Were you previously able to completely expose the head of your penis without any difficulty ...... ?
You could see your own family doctor / GP / PCP who should give you a suitable steroid ointment to use for between 3 and 5 months or so, or they may refer you straight away to a Urologist - initially you should be given a suitable steroid - but be warned, a Urologist would simply want to circumcise you, if you would prefer to avoid this.

about 6 days ago im 46 and have always been able to pull it full back until it went red

It would seem rather unusual to progress to the state it is now within six days - had you applied something to the head and / or foreskin that you had not used before?
If you are still able, continue to expose the head on a regular basis and very carefully clean the head and entire foreskin and make sure you fully dry the entire area, as, once you stop doing this the tightness will worsen. NEVER use hot water on the area, even hot shower water will further damage sensitive genital skin
If it is not looking like it is improving within 3 to 4 days you will need to seek medical attention
