Foreskin tight when erect and can't pull back

I've had this condition for over 1 year. Its very tight when erect and hard to pull back. I've seen a doctor and tried all creams antifungal... etc etc... I've changed shower gel and used warm water to clean it. Ive had a urine test and i do not have an std..... I'm at the end of my tether... Does anyone else have this condition... after sex its sore and sometimes has very small cuts... can not have sex twice consecutive. I am 45 and have never suffered from this condition before... does anyone think it could be a vitamin deficiency? I have no idea and urologist wabts to charge 200 pounds for 30 minutes. please help

    You probably need to be circumcised as did a friend of mine who by that time was in his late 50's. Did the doctor not suggest that?

    The Dr. Could cut the thin skin strip that holds the foreskin to the front , it's right under the head . Mine gradually pulled back in the first few years of sex ( with some pain ) and eventually was all pulled back . I'm surprised at 45 you never had the foreskin pulled back all the way
