Gall Bladder.....Maybe?

have been feeling bad, off and on again for quite some time.  I use the term flare up as it seems to hit pretty suddenly. (seemingly the day after eating greasy food not 100% on that) but when it "flares" it can last for a week or 2, with major bloating infrequent and small bm and discolored bm (orange to yellow tmi, I know but I think it is involved somehow) I have had constant low grade pain in my right shoulder blade for a very long time.  30 mins or so after eating I get nausea...I never vomit...just feel green.   Energy is very low, tired all the time.  I had a nissen fundoplication a year ago which fixed my reflux.  I have seen a gastro doc with this been scoped both ends all is good, blood work is good only thing at all was a low anion gap.  Kidney function is good, sonogram shows normal gall bladder, hida scan shows 87%ejection fraction.  Surgeon says biliary dyskonesia and wants to remove my gall bladder.  I am very hesitant to remove it knowing it tests ok.  I am great with removing it if it will fix me but would be pretty p*ssed off if I get it out and still have these problems, I want  to be back to my old self before this started a couple of years ago.  I am hoping someone has had similar problems or can offer some advice as my regular doctors say nothing is wrong.

    So I have been through a myriad of doctors.  My primary care doc said I had a rare genetic blood disorder called porphyria and sent me to a specialist.  Specialist said I don't have that and ordered an mri of my abdomen as always everything is perfect.  I told him about the pancreatic enzymes helping so he said I have pancreatic insuffieciency and put me back on creon.  I go see my gastro and he runs the tests and I do not have pancreas problems.  I had to go off the meds for a week before the tests and started feeling real bad after a few days.  Back on the meds and feeling better again, still have the back pain and very light colored bm.  I also saw the surgeon who did my nissen and he said my gall bladder doesn not need to come out.  At least I got my vitamin D up from 25 to 52 with lots of sun and 5000 units a day for 3 months.  Anyway I am going to try a bile supplement and see what happens.  I don't want to be on the pancreas meds if I don't need them.  I am about done with doctors, they arent helping and are making me broke.

    Hi! My gallstones did not "show up" on ultrasound. I also thought a HIDA scan showing 88% was normal until I met with a surgeon. He said that 88% was too high and that 20% of the time tests miss gallstones. Anyway, he was certain my gallbladder needed to be removed, so I had laparoscopic removal yesterday. Turns out, I DID have stones and am awaiting pathology to rule out GB cancer and/or infection. Just wanted to share since it seems our test results were similar. Hang in there!

    Still fighting this freaking mess.  Was real sick week before last, went back to primary care doc who ran more bloodwork............Finally something showed up but can't get a clear read on what it means.  My blood lipase levels were 90 on a scale of 0-59.  A few days later I started feeling better again, in fact felt real good, they re ran the test and lipase is normal again.  Per some research elevated lipase alone may indicate an inflamed gall bladder.  Awating the GI doc with this new info.  I hope I am nearing some resolution, I want to feel normal again.

    New GI doc now thinks it is fatty liver.  My mri showed "mild fatty liver"  I am not convinced.  Not sure how that can elevate blood lipase and no other blood work.

      I didn't think fatty liver gave any symptoms? I may be wrong? Have you had a HIDA? They won't give me one as i'm in the UK and most people from the UK on this forum don't get the correct course of action until they end up in hospital!

      HIda scan was perfect, MRI perfect other than mild fatty liver, ct scan perfect.  Still have pain in my back at right shoulder blade that gets worse when I feel bad.  Antibiotics seem to make things better for a little bit.  I don't think it's fatty liver either.

      That is an odd one especially as your HIDA was normal. Maybe fatty liver can cause pain then.

    SIBO testing today.....that is their next idea.  LOL at this point it is absurd buy why not?

    SIBO testing completed results coming soon (not that I am counting on anything showing up ....I am perfectly healthy according to every test, well other than the every meal nausea and right side back pain)  Fatty liver doctor ordered another hida scan, hope to knock that out this week too.  My insurance deductibles reset at the end of the year so I must go for broke.  Interesting enough the fatty liver doctor asked me if I had ever tested for SIBO when I told her antibiotics made me better for a bit.  I will keep updating this thing if anything to help someone else...IF I ever get a diagnosis.

      Did your first HIDA scan reproduce your symptoms, i.e., you actually felt sick during the test?

      first one did not.  I had one yesterday about 12 minutes or so after drinking the drink that activates the gall bladder, I got nauseated.  I told the nurse.  Waiting on results.  Maybe I will hear something monday.

     No, I had a couple of sharp stabbing pain’s right where the gallbladder is but they quickly subsided

    It appears my 3 year health crisis may be coming to an end.  I just got the second hida scan results on my online portal......My gall bladder is bad:

    TECHNIQUE: 5 mCI of technetium 99m Choletec was given intravenously and planar imaging performed. At peak gallbladder filling, CCK was administered and quantitative data was collected.

     

    CLINICAL INDICATION: Nausea and abdominal pain

     

    COMPARISON: 5/13/2016

     

    FINDINGS:

     

    Initial tracer concentration in the liver is prompt, homogenous, and of normal intensity.

     

    The liver appears normal in size, position, and configuration

     

    Gallbladder visualized at 45 minutes post tracer administration.

     

    Subsequent images demonstrate prompt excretion of tracer into the small bowel.

     

    The calculated gallbladder ejection fraction is abnormally low. The percentage is likely not accurate as the gallbladder appears to still be filling at the time that quantitative analysis was performed.

     

    Normal range is >30% at this facility.

     

    Patient reported symptoms after administration of 8 ounces of Nepro.

     

     

    IMPRESSION:

    1. No scintigraphic evidence of acute cholecystitis identified.

     

    2. Findings suggest chronic cholecystitis.
