Genital herpes or something else?

Hello!

43 yo male here. Symptoms started a month ago as a burning pain in pubic area irradiating to a right thigh and scrotum. Few days later noticed a group of round small pimples, looking like vesicles, on the right side pubic area . Photo is attached. Had only protected sex (both vaginal and oral protected) several times during the preceding weeks. Last unprotected oral sex was half a year ago. Never had unprotected vaginal or anal sex. Checked HIV and syphilis - both negative.

Thought that got genital herpes outside of condom-protected area. Started valacyclovir 500mgx2 per day for 5 days. Felt better for some time but then after stop of the treatment the new similar elements came. The skin under them is a bit elevated now on the right side.The very first few elements have transformed to red acne-like pimples. Just started a new course of valacyclovir.

Now have some doubts about herpes.

  • the elements are only on the right side of hairy pubic area. I thought that herpes likes thinner mucous skin more.
  • the elements are mostly grouped, confluent. However, there are solitary too.
  • The outbreak is for more than for a month now. The first elements are still present but look like acne now, not vesicles. I thought herpes rush should resolve faster.
  • Not sure if antiviral treatment helps.
  • Pain is more dull now.

    Can it be some fungal infection or folliculitis? I am confused.

    No, this looks like herpes. you may need to change your antivirals if youve nit had any relief
