Has anybody had a Sphincterotomy?

Hi All, I've been advised to 'seriously consider' a Sphincterotomy following Botox, which doesn’t seem to have worked. Although I'm booked in for a second round of Botox, my consultant advised me to have the 'risky' op The risk he says could result in incontinence and this is what scares me. Has anybody had this procedure, what was the recovery time and did it work, without the incontinence? Thank you.

i had sphincterotomy yesterday and im already glad that it was done. ive suffered 6 long weeks this go around. i had my first fissure sept 2020 and had botox. it works temporarily. my dr said if it didnt work the first go around that there is no point in doing it again. have the surgery. you will be glad you did. i had my first bowel movement after surgery and it already felt better!!! it burned a little but no pain like before.

I had this surgery 3 days ago after suffering horribly for almost a year. i havent been able to work or even get out of bed for 2 months. i was in agonizing pain for at least 8 hours everyday. It got to the point where i hated even waking up in the morning. Here i am 3 days after surgery and am basically pain free. No pain at all during bowel movements, maybe a light stinging for a minute or 2 after and thats it. I cannot believe i waited so long. This surgery literally saved my life. Dont be scared, the recovery for me has been so easy. I havent needed anything more than advil a few times. Just make sure you research your surgeon and get a colo-rectal specialist. Thats key. Best thing I ever did.

Hi all! I knots this is a late discussion but it’s worth a try. Ive been dealing with an anal fissure since before thanksgiving. The pain is horrific at times. Pain that I have never experienced before. I have been using the cream prescribed by my colorectal surgeon. At times it helps but not enough. I had a horrible spasm the other day while driving and it was disabling. The light turned green and i just sat there until the pain calmed down.
Is surgery worth it ? My doctor said it may cause incontinence. She said I’ll be At greater risk bc I have IBD. I already run to the bathroom numerous times a day. Im only 36 and I’m petrified about being in a diaper if I go through with the surgery. I have an appointment tomorrow to discuss botox and surgery. It effects my life almost every day. I am worried I may lose my job over it as well because its hard to finish my shifts.

did you end up getting the surgery ? if so how are you feeling ? i am waiting for the schedulers to call me to set a date for the sphincterotomy i am so nervous and scared of incontinence. but i am done with this pain i feel everyday due to the fissure .
i hope you are doing well now and painfree.

I realize this post is several years old, but going through the replies has helped me to see how others have dealt with recovery so I figured I'd give my experience.
I had lateral internal sphincterotomy and removal of two external skin tags exactly a week ago. The day of the surgery was not bad at all. I was put under general anesthesia and woke up with a little discomfort but no real pain. I was released same day, and I had my first bowel movement that evening. I was extremely nervous about how the first bowel movement would go, but it wasn't bad at all. The cotton came out and there was quite a bit of blood. It was WAY less painful than the fissure itself. I did feel very raw and didn't feel like I had great control of my bowels. I never had an accident, but I did have some leaking. I've had to wear paper towels or pads in my underwear at all times, and I'm still wearing them a week later but it is improving.
I was given narcotics but I never took them. I've gotten by on ibuprofen. Sitz baths are your friend. I recommend several of those a day and after bowel movements. Here's my experience in the first week:
Day 1 (day of surgery): Pain wasn't bad at all. I didn't take any of the prescribed pain medication. Laid on the couch most of the day because it hurt to sit up.
Day 2: Pain was quite a bit worse. Still didn't take prescription pain meds, but stayed on ibuprofen. I had another bowel movement and the pain was still far less than I had before the surgery.
Day 3-6: Still no pain during bowel movements, but I found that I was burning more after bowel movements. I think it was where the skin tags were removed becoming irritated during BM. I would stay sore for 20 minutes or so after BM but then it would pass and I would be fine to sit normally again. I kept on with sitz baths and using fragrance free flushable wipes. Reduced the dosage, but stayed on ibuprofen. I actually felt well enough to drive on day 4 with carefully planning my BM before my trip. I did fine and had no accident.
Today is day 7, and I just discovered this message board and saw all of the recommendations for calmoseptine. I got the Walgreens brand here in the US for around $5, and this stuff is AMAZING. I kept the wounds dry for a full week according to the surgeon's instructions, but the burning/pain after BM (not during) was getting to be too much. I've had a bowel movement today and immediately put the calmoseptine on after cleaning well with wet wipes. This has reduced the post-BM burning by probably 80%!!! If you're reading this before your surgery, do yourself a favor and order this stuff to have on hand. It's wonderful!!! I see it's on Amazon, but I got it locally at Walgreens.
Would I do the surgery again? Yes! The reduction in pain during BM is tremendous. I had not realized how much the chronic anal fissure was weighing me down. I think I was depressed from constantly knowing I was going to be in pain and didn't even realize it. Yes, I still have a little leaking, but it is improving and I think it will eventually resolve itself. I am not worried at this point.
I have a follow-up with the surgeon next Friday. I saw a colorectal surgeon, and I'd definitely recommend seeing someone that is very familiar with this procedure. My surgeon said he's done hundreds of these procedures and very rarely sees permanent incontinence. He said it's even more rare in males because of our muscle anatomy.
I've been taking fiber daily, but I am no longer taking Miralax like I was before the surgery. I feel like I'm going more often and having smaller bowel movements, but I've been pretty regular. I've resumed a normal diet and just drink plenty of water with the fiber.
I was so nervous about this surgery, and yes there is some discomfort, but it's probably not nearly as bad as you're thinking it will be. I was in so much pain before the surgery that I was desperate for relief. I tried nifedipine and nitroglycerin. Those helped a little but it always came back. I skipped the botox and went straight to the surgery with the recommendation of the surgeon. I'm glad I did because I am no longer dreading every trip to the bathroom.
Find a good surgeon, and I think you'll be fine. I suffered for nine months and wish I would have gone to the surgeon months ago had I known it would be this much of an improvement so quickly. I told my surgeon if he fixes my problem, he was going to be my favorite person and I'd send him a Christmas card. He said, "Well, I think I'm going to be your new favorite person," and that's the last thing he said before I went into surgery. By my follow-up next week, I think I'll be doing even better. He's helped me a lot.

Yes, I'm replying to my own post, but I thought I should update and let you know the last two days have been HUGE progress! This is two days after my original post so 9 days after surgery. I have had no leaking at all today, and I had my first bowel movement with NO pain during or after!!!! I've not taken any ibuprofen or even had to use the calmoseptine ointment! This is a huge success for me! I have been taking my fiber and drinking water religiously. That's my best recommendation for anyone that has the surgery.
If you're on the fence about whether to have the surgery, do it! I am SO glad I did and it's over. It really wasn't that bad. I had suffered for nine months. This week after surgery was not anywhere close to the pain that I experienced before the surgery. I know it's hard to think that it's not bad when they're literally taking a scalpel to your butt, but it's really not that bad. Would I do it again? Absolutely! I'm pain free!

Hi, i had Sphincterotomy 12 years ago after trying every cream and ointment under the sun, its no walk in the park or at least wasnt for me. I was totally unprepared for the post-op pain which was to follow but in essence had no choice since everything else had failed to heal me. In a nutshell be prepared for pain, get on top with pain meds and stool softeners and dont expect instant healing, take plenty of warm baths especially after BM and try to stick to a healthy diet.

After 30yrs - it is the only thing that has ever worked - and has been near miraculous
Despite awkwardness/embarrassment of administering this intervention to yourself it is effective and free

Hi all im hoping for some advise, im in the UK.
I had LIS approx 5 weeks ago after years of hemmaroids inhad these banded then i had botox as ineas diagnosed with anal fissures.
I am a 36 year old female fit and healthy and a decent diet.
I was really bad after surgery for about 2/3 weeks signed off work ans didnt do a lot.
I am now approaching week 5 still taking laxido every other night a gentle walk and healthy diet. However I toilet in the morning and bath straight after but during the night early evening i feel itchy and uncomfortable, inside? Is this normal?
so fed up willing to try anything at this point im
miserable and its affecting my family.
ive even paid for intolerence tests and cut out dairy and beef as this was not recommended. .
