Has anyone had the Hal-Rar Surgery for grade 4 hemorrhoids / bleeding Piles?

Hello, I live in Australia. My problem is piles/hemorrhoids which I've had for a very long time. Since I turned 40 they have been bleeding really badly. For the last 2 years if eat anything processed my hemorrhoids bleed. Sometimes bleeding 3 weeks at a time. I have seen Drs, Specialist, surgeons, dietitians etc, and have had lots of test done. All good, most say it's just hemorrhoids eat more fiber, fruit and vegetables or buy Ansol cream. I was a vegetarian for 3 years, and took psyllium husk daily and exercised. I don't smoke or drink alcohol, so I thought I was already doing all the right things. I started thinking maybe the potatoes, bread, pasta and processed vegan food I was eating for 3 years made this hemorrhoid problem worst because my piles didn't sqirt blood and bleed bad like this before. Or maybe it's just turning 40. So I went back to eating meat and vegetables, cutting back on the carbohydrates. Still no improvement in the hem area. I saw a surgeon who said he would cut the hemorrhoids out. It's a 12 month + waiting list to get this done in a public hospital here in Australia. So I've been researching other options and found the Hal/Rar treatment. Has anyone had it done? Anyone in Australia on here? Does it work? Any info is appreciated, Thanks.

    hi annie, i live in Australia too . I have heard of hal rar before and was researching this procedure as well. Did you went for first referral to the specialist in the public system which then they put you in another 12 months waiting list for the traditional surgery?

      Yes, that's correct! The waiting times are 6-12 months for traditional surgery. But I ended up going private. I had the Hal Rar surgery and all is good now the bleeding has stopped. I was in pain for 2 weeks after surgery. It wasn't that bad because I was taking pain medication every 4 hours for 5 days.

    So I had this surgery last week here in Melbourne here is my experience

    Day of the surgery

    Woke up after the operations and felt absolutely no pain. Just edgy from the anesthetic.

    I went home that night - and threw up in the car... just the long trip home and the drugs i guess

    Surgeon tells my wife the had to stitch a little lower to get all prolapse and that i may hurt. Warm bathes and Endone should help

    Day 1 - 4am

    I woke up - no pain - just a dull ache

    I cant pee - that's weird. Numbness in genitals ! Trying not to freak out

    7am - finally pee - phew

    The dull ache of needing to go to the bathroom gets strong - but nothing there

    Rest of the day is a dull ache - no real pain. Haven't even taken a pain killer - this going to be easy

    Day 2 - 9am

    First BM - OMG - i feel like im being ripped into 2

    Pain off the charts... then its goes just as quickly

    the Actilax (Stool Softener)has Lactose and Im Lactose intolerant ! - so you do the math. It freaken hurt and was explosive

    12pm - need to take painkillers - clearly the local is wearing off. the numbness in genitals has passed.....

    2pm - KILL me NOW the pain has started !

    The Endone isnt doing Jack

    The warm bath is the only relief i get

    Taking Nuromol, Endone, praying!!! nothing is helping

    Finally the wave of pain goes.

    Day 3 - Wake up at 6am in pain

    One Endone does nothing, take a second and Nuromol all at once - Nothing

    Make an appointment with GP - prescribe panadeine Forte

    I send an email to my surgeon

    Take the Panadeine Forte and im out like a light

    10am to 5pm

    Speak to my surgeon who says i might need more Endone

    Pick that up from the pharmacy (Sunday evening) and alternative non Lactose Stool Softener

    Panadeine every 8 hours and Endone every 8 hours

    The pain is strong - about 7/10 but under control

    BM hurt alot and the urge to strain - but doing my best

    Day 4 - Wake up with 5/10 pain

    Take Panadeine Forte and Endone 4 hours apart

    Pain has now slipped to 2/10

    BM still hurts

    Would I recommend the Surgery?

    Too early to tell as i dont know the end results

    I will say its not Pain free... im not a wuss! But if the surgeon operates in the zone with nerve endings it will hurt.

    I think my surgeon was really good and the team that worked with him where really nice..

    Ive had issues for 20 years so Im hoping this a a medium/long term solution

    If you are reading this and are in pain, I genuinely feel for you.

    Call your GP and see if there is an alternative pain relief for the operations

    Banding hasnt worked for me in the past.

    all Up I was $500 out of pocket

    Hopefully Day 5 is even better !

      Yeh the pain can be pretty bad at times. I took heavy pain meds before I used the loo. Passing wind feels like razer blades. And the anal spasms yikes. But we get through it. I was pain free at 2 weeks. I shopped around surgeons in Sydney and was quoted anything from $4000 - $6000 without health insurance.

      Hi rob,

      can you please tell me how long it took the prolapse to disappear? I had hal rar 3 weeks ago but I still have prolapse after bowel movement. Im worried i might have tore the stitch when i strained

    Yes, I had the surgery halo halrar 3 months ago. It was all good, I had day surgery. I had a wonderful anesthesiologist who prepared me with prescriptions for pain meds. I took pain medication for 5 days. I couldn't sit down without pain so I was laying on my side or stood up for the most part of 2 weeks. After 2 weeks I felt fully recovered. Then after 3 weeks I had swelling and looked like the hems had fallen out again. The Dr said it's skin tags, no hems. So now it's been 3 months and was feeling really good. Then today I had spotting blood on the paper, I look swollen again. Bummer 😕

    So I look on this website a see all the replies to my old question. Thank you everyone who has shared there experience with me. I didn't know anyone had I had replied.

      Has everything settled down now.? My biggest worry is that the hemarroids come back and I have to do it all again. It is 3 weeks today since I had the surgery. I am still using some painkillers and heatpacks but it is slowly getting better. I too have skin tags which i have been told will disappear over time.

      Hi Cynthia, Yes I'm better now after the panic attack of seeing blood. What happen was I had gotten constipated from cutting back on laxatives, and had some blood spotting. So I'm back on a higher dose. I'm also on a strict diet so I don't know why I'm struggling with this problem. I only had the halrar done so my pain recovery was only 2 weeks, but 4 weeks for my bowl movements to be under control. I have small skin tags left over from banding them years ago. They are still there. And the big one I call the flap is what I want fixed. I though when people had the hemorrhoidecomy surgery everything would be cut out. But I'm reading on here some people still have skin tags. I hope the hems don't come back too. But if they do I will have to do it all again.

      I've been suffering with Hemorrhoid for almost 10 years and I found this forum and really interested in Hal rar procedure. just wondering could you please tell me the name of your docter and which hospital? I live in Canberra and there is no docter who can do Hal rar surgery here. 😭

    Hi everyone!

    Glad to be part of this forum.

    So i have heard a combination both internal and external hemorrhoids in the last 15years and been managing it lately with a botox injection .

    January 8th 2019 ,had a BM and usually would take a hot shower and push up my hemorrhoids but this time it failed and kept falling back.This stayed out for a week and got thromobosed which caused bleeding that led me to emergency on sunday morning 13th JAN.

    BTW,i live in saskatchewan canada where health insurance is covered by government .

    So with the pain and bleeding i was examined almost immediately and the general surgeon on call confirmed i would need to be operated same day.

    Long story short ,no details of what will be done on me but at 6pm ,i was wheeled into the OR and placed under general Anesthatia which was all i remembered until around 8:30pm when i became active again.

    SURGERY DAY: Was asked not to eat and didnt feel any pain until the anesthia wore off.

    DAY 2: Around 9am,this crazy pain kicked in and my nurse stated to place me on hydromorphine with tynenol.

    Part of the crew during the surgery came for a follow up and noticed their was a swellen which he wasnt sure if its another hemorrhoid prolasping.

    I asked if they could take it off as well so i can go through this terrified experience once but he said no.

    Day 3: Pains continue and discovered my hydromorphine was 1mg hence i increased it to 2 tabs as needed to help whilst using softner to make bowel movement easier.

    BM happened on day 4 and wasnt bad .

    its now day 6 and the prolasped stuff is presently a concern to me as i dont know if its hemorrhoid or just skin tag .Can someone please share their experience if it will shrink and how long will it take?

      Hello, I'm 3 months post op Halo hal rar surgery for internal and external hems. 3 weeks after the Halo hal rar surgery my hems fell out, after being nicely stitched inside. They look large to me and feel like hems but without the bleeding. The surgeon had a look and diagnosed them as just skin tags. I asked if he could remove the skin tags but he said no as removing them could cause healing problems and infection. I don't feel like the skin tags are shrinking any.

    Hello I am 36 years old and I had RAR surgery two and half weeks ago. I live in Paris so I had the surgery here. The pain 2 days after the surgery was pressure that felt like someone was pulling something inside my ass with an immense power. After the bowel movements in the mornings, I felt relief. But first 3 nights, I couldn't sleep right because of the gas and the pressure inside, real nightmare. One week later, after I went to toilet, I felt that the pain has gone away.

    2 days later that bowel movement, I felt the hemorrhoid came back because of that spasm feeling, exactly like before when I had the hemorrhoids. After an examination with my finger, what seemed to be pushed inside with RAR surgery, came back and now it is exactly the same size before the surgery.

    I have an appointment in 3 days with my doctor. Maybe it is me worrying over something that will go away in time or maybe the stitches inside couldn't hold the hemorrhoids. But I don't feel any better than before, I still can't sit without that throbbing sensation down there. I am starting to think if my stitches snapped or something, if that is even possible. I had no bleeding except the first BM.

    thought i would share my experience for anyone suffering from grade 4 prolasped hemaroids. im now post op for hal rar, im a 47yo male and have experienced prolasped regularly on each BM which was pushed back in by hand. in recent times the hems would swell and come out on any physical exertion to a point where even when i was laying bed they would pop out.. however it was always manageable until now. i research treatments and found dr ahmul had performed successfully 1000 ops in perth.

    prior to the surgery i had to get an all clear for a colonoscopy which i did to rule out any other issues as i was anemic from bleeding from time to time. at colonoscopy the doctor also performed a gastropy to investigate any stomach issues. i woukd recommend if you are doing the colonoscopy to also get this done since you are in the hospital anyway, and effort is another 10mins - luckily i came back with no issues so the hal rar surgery was scheudled.

    op day

    checked into hospital and was told it would be overnight stay, i recieved two enemas which made me clear my bowel but left me quite raw and swollen.

    the op itself was quitr easy and when i woke up it was all done. the dr said i was quite extensive a grade 4 and if there was a banding above that i would be it. he had done 5 circumferential hems.

    day 1 post op

    couldnt wee

    couldnt pass bm

    had 6 bags of movical over a few hours

    was told i had to wee and do bm before being able to check out.

    i was able to wee about 12pm and finally did a bm at about 5pm where the insert was passed.

    i was allowed to go home and given

    tanpandental pain killers

    oxicone strong pain killers

    lactuose laxatives

    celebrex anti inflame

    metrogyl which is antibiotics

    panamax

    day 2

    took 2 tabs of tanpandental every 6 hrs

    had serious urinary retention

    managed to have a small stream to avoid a cathater

    at 10pm my bladder was bursting anf finally peed for along while

    throughout the night continued to pee - i think my body was expelling the various meds

    i seriously think the opiod made me urinary retentive so stopped this altogather. only pain was controlled by panamax

    did about 3 bm which all were smooth due to a tablespoon twice a day of laculose

    pain after each bm was a 5 - hurt like hell

    stinging feeling which subsided after 5-15 mins

    day 3

    bm about 4 - pain still unbearable after bm resorted to taking one tapandentol to sleep

    urinary retention on and off - nurse told me could be related to the severve pain which i beleive could be true howver opiods def dont help.

    day 5

    still recovering taking epsom dips now which help

    i think will take me another good week or more to feel better.

    hope this posts helps. overall i think i made tge correct choice in dr and procedure and hope to update at 12 weeks.
