Having a flare.

PMR 5 years down to 6.5 mg FLARE Pred upped to 8 one day . No relief(left shoulder blade/left hip-can hardly walk)Upped to 15 mg today Little relief. continue next several days at 15? Or up to 20/25? Have been reading forum last 5 years and should know this. Thanks

    This is my 9th year, been on 2mg past 5 yrs I try to taper and hip joints get terribly painful, you might need to have Dr. check your sed rate, going to 15 mg of pred. should have helped! I am very careful with pred. The side effects are terrible. Hope your having a better day.

    Hi Roberta, I would stay on 15 mg for 3-4 days. If not increase to 20 for 3-4 days, inflammation not under control increase to 25. Remember to keep a record as to your increases and reverse for your dosage decreases. Good luck getting the flare under control, may the rest of your PMR journey be smooth. 🙂

    I am so sorry to hear of your troubles; you would think that after 5 years you should be saying "goodbye" to this affliction?

    I am only in my 3rd year but sound very similar to you in that I have recently had to up to 15 mg (from down to 10 mg). My left hip has been so bad it's hard to walk! I even tried upping to 25 mg with my doctor's approval (where I first started) but it did little to help.

    I think we may be suffering from the fact that the pred weakens our muscles? I visited an exercise physio recently who gave me some exercises to do. I think they are helping.....

    Try to get some tolerable exercise (gentle exercise in a pool is wonderful; sideways walking etc) and also maybe try paracetamol/osteo as it may be that good old arthritis is contributing?

      If it is just one hip that is bad - has anyone considered it might be bursitis? Very common in PMR and more likely if it is just one side that is troubling you. But an x-ray is always a good idea to be sure it isn't something else.

    Is the flare in response to a decrease in dose do you think? If so, usually adding 5mg to where you flared for up to a week to 10 days and then drop back to just above where you flared should work without losing all that hard work reducing! 8mg wasn't enough, 10 might have been.

    But you should stay where you are for at least a week to give it a chance to really work. Try not to go back to a higher dose, be patient. Often with flares you don't see the magic massive response you had the first time. Then don't drop straight back to the 7mg, try 2.5mg every few days to 10 and the 1mg every few days to 8mg. Always providing that works - if this is a flare due to an increase in the activity of the underlying autoimmune disorder you MIGHT need more again.
