Headache going on three weeks

Background: 37 year old male, without a history of headaches. Back in January (4 months ago) I had back spasms, but recovered in a few weeks. About three weeks ago, I had what felt like an earache, but that subsided. A few days later, I started getting headaches. It felt like pressure in the back of the head, and my ears/hearing was off. This progressively got worse, so I decided to see a Doctor. The Doctor said I was having tension headaches, which I believed because I have a stressful job as a psychotherapist. For about a month prior to the headaches, I was working 50-60 hour work weeks, so it made sense. I've been taking OTC, and icing my head several times per day. The intensity is gone, but a mild headache remains, especially when I bend over. Overall, my headache has lasted about three weeks now. Any thoughts? I deeply appreciate it, thank you smile

It could be a migraine. They are more painful when bending over. They can also make you ears feel slightly blocked with some pain.
