Headaches every day from car accident 1.5 years ago , help

i was in car accident in march 2017, had concussion , i was off for a year from work due to car accident because i had a hip replacement from the accident as well. i went back to work and noticed that my headaches, neck pain has gotten so much work, i even cut my hours at work for a few months. however, its debilitating . i am exhausted from constant pain, i have been and continue to go to physio. i have had massage, movements to my neck, shockwave therapy and needling and nothing has helped. why doesnt my doctor think a referral to neurologist is not appropriate. i take 2 tylenol 3's at a time at work even and no relief. any advice would be greatly appreciated!! I am 53 and have years until i can retire. i am losing hope
