Help? Hiatus Hernia with Severe Acid Reflux.

Over 3 years ago i found out i had a sliding hiatus hernia after months of coughing and then that horrible feeling you get that something is stuck in your throat. I was put on Lansoprzole 30mg but would have only been taking it as and when required as acid reflux didnt seem too bad at the time. Well fast forward to now and it has gotten horrific. Ive actually so much anxiety about going back to the doctors incase its something more sinister. I am taking the stomach tablets daily and have started to take two tablets, none morn and one night and drinking serious amounts of gaviscon. I dunno what has suddenly triggered this, but thinking back about 7 weeks ago, i remember laughing really hard then suddenly i took this massive pain around the area of hernia, it felt as if my insides were twisting inside out, it was awful. It setteled after 30 mins, but i dunno if that caused damage, but since then, the acid reflux is unbearable. Nothing is helping whatsover. Has anyone had anything like this pain before, caused by laughing, coughing, exercise etc? If so was it the hernia moving do use think? Could this be the sudden increase in acid? Any familiar stories appreciated.
