High fever 41.5, headaches

Hello everyone, dear formuchans. In November 2018, my younger brother, at that time he was 9 years old, had a high fever, it was in the range of 39-41, 5 of the symptoms were only severe headaches, then his weight was 41 kg. The temperature lasts for about a month, then disappears and after an indefinite time appears again . It's been almost 3 years , now my brother is 12 years old . During this time, he gained extra weight and now weighs 55 kg. They were examined in different clinics, but no doctor made the correct diagnosis . Every 2 months we undergo tests such as (heart, diabetes, etc. completely all human organs ) there was no temperature in the summer, and since September, 41.3 began again. I hope that there are people here who have faced a similar situation ( high fever, headaches, weight gain).
