High pitch ringing in ear, long lasting headaches - ENT/MRA finds nothing wrong

I am a 21 year old south asian male 5'10 and weight 120 pounds. for context (not sure if related to the problem) I came back from vacation in Punta Cana, Dominican Republic around 3 weeks ago back to New Jersey. In the domincan republic I stayed at a resort full of all Americans so it was a high class resort. I swam in the ocean as well as pools. I also got a lot of mosquito/bug bites. I was stuffy on the plane ride and I was also listening to music through earphones. I noticed once I landed something was wrong with my ears. specifically my right ear, as well as pressure in my head. At first I thought my ear was blocked, but after a day or two I could hear perfectly fine yet I felt some type of fullness in my ear and head. Skip past a week later, I was having minor symptoms but I decided to smoke a cigar. Right after I started hearing an extremely loud high pitch noise in my right ear as well as a powerful headache. I experienced double vision for a very short time and the problem was so bad I was about the call the ambulance but decided not too. I went to a neurologist and he says he doesn't feel that anything was wrong but gave me a script for a MRI and MRA of the head just because I felt worried as I have never felt these symptoms before.
A couple days later I went to the ENT thinking it was an ear problem but they checked out everything as perfectly fine. My hearing test was perfectly normal as well. I had minor issues but the ENT made me feel like I had none at all so I decided to smoke again since I was bored, the same symptoms came back extremely powerfully. This was on March 30th. Ever since then I have had a pretty strong pounding headache in which I feel as if a heartbeat is in my head. Mostly towards the back of my head as well as the sides. I also hear a high-pitch ringing in my ear. At first I thought it was only the right ear but now I only hear it in the left. So it seems to be high pitch ringing in one ear at a time. I started thinking I have a problem in the brain after searching my symptoms and began to become really worried. I had an MRA yesterday and all results came back as normal as well. I got blood work done today I'm waiting to hear back results, my parents think I may have lime disease. Because I am so young my insurance won't approve me for an MRI which is basically the last thing I need to get done. So far I have no figured my issue and neither has any physician. The problem has been going on since I got back around March 15th or so. It has gone from mild, to severe to mild back to severe. Smoking seemed to be a reason for it becoming more severe. And after smoking on March 30th I still feel some of the symptoms. Yesterday my head was pounding the entire day, I felt as if my brain was too big for my skull to fit. Since yesterday it has gotten slightly better, my head pounded a little today. It isn't currently, but I still hear a high pitch noise and have some type of disturbance in the back of my head. Also I have been feeling as if my brain has been cracking as it keeps making cracking/crackling noises and I keep feeling the need to stretch my head and neck. My brain/head has never really cracked as it has. I have no history of headaches or any brain problems. As far as I can remember I have no serious health problems what so ever besides being really skinny and I do have a pretty bad diet. I have no existing medial issues. I'm being driven a little crazy trying to figure out what I have as sometimes I feel like I have a brain cancer or tumor but that might just be me thinking the worst. I really want to find out what I have so I can move on with my life as I left university this week because the problems were getting hard to deal with. I would really appreciate your help as it as I am suffering from stress and anxiety specifically because I don't know what my condition is nor the severity of it. If I can find out what I have I would feel so much better, even if you can give me a suggestion of what doctor I can go to or if I should get this MRI. Because I will have to pay out of pocket as my insurance won't approve it. Thank you.
