How do I manage working with Crohn's?

I'm really struggling at the moment. My Crohn's has been linked to a clotting disorder. Over the past few years I've had a PE, MI, clots on my spleen and a TIA. I've been in hospital twice with pain and vomiting from two flare ups. I'm self employed, in April last year following the TIA and crohn's diagnosis I applied for benefits as the fatigue, anxiety and joint pain were overwhelming, I was told I was fit for work. I've since returned to work but I'm really struggling to get through the day. I also have a cyst on my ovary and a prolapsed bowel. My stress and anxiety levels are really high all of the time. I feel like a ticking time bomb and I can't cope with the unpredictability of flare ups and clots. I'd just like to take a few months off to rest and reassess my life which I have no time to do at the moment because I'm either working or sleeping. My funds are really low as I took time off following the TIA and I've had to reduce my clients. I'm frightened we'll lose our house, and the business, any advice on how to improve this situation would be very welcome.
