How fast does it damage happen in the bladder from BPH or retention problems?

I have been taking alfuzosin 10 mg for the last year. However its effectiveness has declined so I doubled up alfuzosin twice a day. The pharmacist said that's not recommended. But it seems to working better though not great. I don't want to take any other medications because all of the others seem to have terrible side effects. Lastly, I noticed recently slight pain in my bladder region. I'm not sure if it's all the stress the bladder has been in cheering for the last year and a half with retention problems. I'm trying to get a new urologist. Because the one I have only does old-fashioned Turp and doesn't seem very concerned. He doesn't see the point I'm getting an ultrasound. He just keeps saying got to do the Turp.

I had BPH for about 9 years, half of that time I was on Flomax, caused RE, switched to Alfuzisin and recovered most of my ejaculate, my ultrasound and MRI show I had about 400-500 ml PVR, also MRI shows some trabeculation of the bladder, eventually I had FLA with Dr.K, all my BPH symptoms disappeared, I went from IPSS of 25 to 1, score one only because I have to get up once to urinate overnight. I was advised I may have to use self catheterization after my FLA to rehabilitate my bladder, turned out my current PVR post FLA is only 20-30ml, so no need to rehabilitate my bladder, seems to be working fine, this is my case, everyone is different, so my suggestion, establish what is your PVR, see if you have any trabeculation, from my experience it takes years to damage your bladder, longer term you have to have some procedure if drugs don't work, BTW, I never had full retention or had to go to ER

oo so the issue is how much are you retaining? you can have damage from over retention your bladder gets weaker and weaker and the muscle gets stretched. A sample size of one...I had BPH symptoms for years and just lived with them. I had low PSA and annual rectum prostate check by GP and he never noticed enlargement but last year thought he felt a nodule so went to Uro. He did several tests and nothing to the nodule but discovered significant retention and blockage despite only a 30cc prostate. Failed Urodynamics and medicine did not help. He offered bipolar/button Turp but was not sure how successful the results would be so I self cathed for a year until I just got tired of doing it. I was not able to void more than a dribble on my own and rarely felt a strong urge but went ahead with the surgery since deductible and out of pocket met. I had surgery at 10 AM and was on my way home a few hours later. Recovery has been straight forward and appear to (based on one attempt) to ejaculate though it was not forceful it came out as expected so will see how that progresses. I am voiding 300-350ml each time and PVR is less than 25ml. I don't know if the self cathing helped or would have seen same results either way but am very glad I did the surgery and shows how much removing the blockage can help. Good luck.
