The size of the prostate is usually determined by an ultrasound scan.  If the insertion of the catheter was difficult, someone might infer that the prostate had grown back, but that would be a bit of an educated guess.  He needs to see a urologist for an assessment.

If the prostate is enlarged a great deal there would be considerable resistance on inserting the catheter. It would appear that the bleeding was caused by the nurse forcing the catheter through the urethra to the bladder. Presumably the bleeding should cease and he will be OK.

The could be other causes than the prostate being enlarged but there is probably some obstruction. As Glenn 77 notes a scan or alternatively a cystoscopy  would be needed to be sure that obstruction was the prostate but as Glenn says it is an educated guess especially in older people.
