I hear clicking and popping in both of the shoulders when I move them upwards.
This doesn't cause pain, only the sounds but sometimes when I use my arm to throw something with it, I feel extreme pain for a minute-two and can't move it (it's like it all of a sudden frozes).
I haven't experienced something like this for a few months now.

Your symptoms are something I experiences: noises in the shoulder when raising the arm, pain when throwing.
The clicking noises might be dismissed but when I went to the ortho specialist he said that was my labrum. And they confirmed not only labrum damage, but rotator cuff damage. It's something that progresses and gets worse.
What concerns me about your symptoms is not only the pain but lack of ROM. You need to go to a specialist who will hopefully order an MRI to definitively find out what specifically is going on with both your shoulders.
I would go to your primary and ask for referral to an ortho and make sure he orders an MRI of both shoulders. A physical exam by an ortho or therapist can't determine what's really going on like an MRI can.
