    Your description and the appearance of lesions on the shaft of the penis are highly suggestive of genital herpes- there is pain, underlying redness and the lesions are grouped together. The ones on the scrotum are also suggestive of herpes, but appear to have ulcerated. It's probably academic, but I'm assuming the scrotal lesions preceded the penile ones; i.e. they are in the later stages compared to the penile lesions?

    You should try to see your doctor or local STD clinic and get the lesions swabbed for herpes (HSV) DNA to confirm the diagnosis and also get checked for other STDs. If you are seen within the next 48-72h you might also benefit from aciclovir medication but if not, take some paracetamol or get some lidocaine gel/cream from your local drugstore or pharmacist to ease the pain.
