I have bph, large prostate and rising psa

2011 psa was 3
2014 was 3.9
2015 was 4.2
2015 was 7
2015 was 5.4
2016 was 5.9
3 in 2017 were 6, 6.8 and 6.7
I have just recently had my second MRI scan first one showed nothing this one has shown 2 Lesions that are pirads 3. Previous biopsy shows no cancer. My prostate size is 80-90cc I am 51 years old.
They are considering a template biopsy or a targeted transperireal biopsy.
1/ after reading my diagnosis does my psa level seem ok for my prostate size. (I have been taking saw pelmetto for 3 years) ?
2/ should I be worried to much about cancer?
3/ can anyone advise on the 2 biopsy’s above?
4/ should I have a procedure to relieve my bph symptoms?

When you have family history on fathers side you should be vigilant. Its all about the aggressiveness. I know many guys in watchful waiting due to low agressiveness as measured by Gleason score. If really aggressive I would get two or three opinions. Embolization being used on other cancers has promise for prostate if aggressive and caught early. Even get other opinions on the biopsy score. This can vary. I have friend who didn't listen, had a Gleason of 6 and had a bunch of lymph nodes removed ( way to aggressive treatment ) and got same thing women get from BC surgery of lymph. I think it is called lymphodema. He had totally removal and regrets. He could have had watchful waiting for a number of years. Nothing but ED for him unfortunately. Makes for some sadness in life. Don't settle for way of life. My PAE was life changing. Most any consultant is going to say have a biopsy as they play it safe for liability reasons. Your a case that charting the psa every 3-6 months can be of some value. I would not take more doses of meds. Bad for sex life. I don't remember prostate size but I would do the following. 1) If prostate >80 do PAE, 2) if 40-80 look at Rezum, FLA and maybe PAE depending on where in the range and symptoms. Good luck my man, but don't expect a consultant to help all that much.

I was unaware of the link between prostate size and PSA or Maybe I forgot. Any my prostate is 125 cc and my PSA is 1.4. I must be an anomaly. I am taking Firnasteride which I'm told halves you PSA numbers
