I have had 'itchy bottom' since my mid-20s (I'm nearly 40 now). I have had all the tests and the only thing they could say was "it's a bit hairy down there"! The hospital consultant eventually prescribed Dermovate, the idea being to break the 'vicious circle' of itching and scratching. It worked like a charm.
I find that food like curry will start the itching off but a tiny amount of Dermovate for a day or two will stop it and then I won't have to use it until the next time. The only problem I have is that Dermovate seems to have quite a short shelf life, so I keep having to go back to my GP who doesn't like prescribing it. I just read that Dermovate shouldn't be prescribed for 'itchy bottom'!
I recently tried Germoloids but it made things 100 times worse.

I was on holiday and ran out of Dermovate, so went to a German dermatologist. He spent ages 'inspecting' me and talking about the problem. He said that if Dermovate hadn't worked in the last 15 years then it wasn't likely to work in the next 15. He prescribed me various lotions and potions and said to come back in a week. As I was leaving I said 'and shall I keep using the wet toilet paper'. He threw up his arms and said 'well thats the problem'.
I've stopped using wet toilet paper and instead have to wash my backside after I go to the toilet. Now I know what bidets are for! Not owning a bidet, 1 minute with the shower head has to do.
I'm essentially 'cured' now. If I don't wash then the itching comes back, but I have a mild steroid to stop this. So the key is to wash myself (without soap) and keep the area clean, and make sure I dry myself properly afterwards.

The cream I used was called Dermovate. I just looked in Wikipedia (http://en.wikipedia.org/) and it says Dermovate is a very strong steroid and Benovate is a medium-strong steroid.
