I have started Nalmafrene

Hi Joanna - thanks for coming back to me,

I started Nalmafrene on Saturday and had the worst experience, I had been out on the friday and had a few drinks then met some friends at the Rugby on Saturday and satrted drinking I then took a tablet and continued to drink after about 2 hours I literally could not function (i guess this the waitiing time period) I have never felt like that in my life. I then was frightened to take the tablet on Sunday, I did, and the effects were again aweful. I drank 4 pints on sunday and a massive hangover yesterday I took another pill and it was not as bad, then this morning I feel fine.

My intention was to take a pill every day as I am an every day drinker and keep reducing the amount I am drinking to a point where I am not drinking at all during the week at least, Is this safe to do as i have reduced from average 25units per day and am now at 12 units per day.

What would be my best course fo action.

    Paddy,

    This method is very, very successful when done correctly so I will firstly just recap on how you really should be looking to do this, to reassure you.

    At the first sign of a craving or urge to drink, take the tablet and wait 2 hours, then drink mindfully.  Recognise that the usual compulsion to continue is gone and then look for ways to chip away at your habitual drinking.  That can be anything, such as putting the glass outside of arms reach, so that you have to actually move to get it, alternate with water or soft drinks, etc etc.  Whatever suits you.  Drinking was so difficult to control before because the endorphin rush was driving you to continue.  This isn't a magic pill - it just stops that endorphin rush and helps to weaken the pathway in your brain that drives you to drink.  Along with the pill, you can help make this smoother by working with the tablet.

    It sounds like you have a good idea of where you want to be with not drinking during the week at least, and given compliance and patience, you will get there.  Each little bit of success on this method propels you forward.

    In terms of side effect issues, I think this is definitely part of the reason why you felt so bad.  It is possible that is being coupled with some alcohol withdrawal symptoms because your drinking has been halved so quickly.  If you are struggling with this, then increase the amount you are drinking and then work to reduce it down a bit.  I am not sure how you obtained the nalmefene, but part of the criteria is that it should not be prescribed to anyone at risk of alcohol withdrawal.  Hopefully your doctor checked you out for this, as it is usually not prescribed to any man drinking more than 9 units a day - just to ensure they don't put themselves at risk of withdrawal/seizures.

    The ideal way to start this method is to take a good meal about 30-60 minutes before taking the tablet.  And drink lots of water - with the tablet, during the wait and several times before bed.  This helps line the stomach and helps to ensure that side effects are kept as low as possible.  I usually also suggest that until someone gets adjusted, they restrict themselves to being at home for a few days.  No-one likes to be feeling rough if they are not in the comfort of your own place.

    However, it doesn't say all this in the Patient Information leaflet, so many people go what you have gone through!

    It usually takes around 6-8 tablets to adjust fully to the Selincro, getting easier each time.  The worst is over for you!

    So, please make sure to take that meal and drink lots of water and you should be through this very soon now.  Then you can concentrate on recovery and making the moves towards where you want to be.

    This medication does seem to knock people out quite a bit, but it doesn't directly interract with the alcohol itself.  Many people do seem to feel the affects of alcohol quicker, but that might just be the 'real' effects of alcohol without the endorphin rush making it feel better.

    And finally, the short term discomfort of this medication is well worth the long-term benefits that it gives.

      Thanks Joanna - thats very helpfull and I will take a look a the article...

      I am attending group meetings for help and yes my doctor did prescribe this, I had 10 units yesterday and feel fine today so will keep this going for now or until the end of the week dependant on how I feel. then reduce again next week, already I am feeling positive effects and can see that the pill will help just need to bear with it.

      Hi Joanna, I have had my 4th tablet now and got to say that last night was the best night I have had and I feel good today however I ony drank 8 units last night and felt like I had a hangover this morning is this due to the pill?

      Also I am planning to go out on Saturday for the last day of the 6 nations and the usual plan would be to have a few to drink is it best that I do not take the pill and have a drink will this hinder my progress? I am just not sure..

      Absolutely NOT!

      Now you have started this method, you must, must, must remain compliant.  You are trying to teach your brain that the rush from the alcohol is not going to happen ever again.  You can't do that if you let it have that rush - it will get mixed messages.

      I would say you feeling a bit rough will be partly down to the pill.  It often takes around 6-8 tablets to be free of any side effects.  Also, without the usual endorphin rush to make things feel a little 'better', it could also be your body actually feeling more effects of a hangover (there is no buzz to dull that hangover feeling anymore!).

      Otherwise, you are doing great and this is a great indicator that this will work for you long-term and your cravings for alcohol will be extinguished over the coming months.  But, I can't stress enough, in order to make the next few months as smooth as possible, compliance to taking the tablet and waiting before every drinking session, is key.

      Without compliance, you are risky a very bumpy few months as your cravings fight to try convince you to not take the pill and let it feel that buzz again, and the worse case scenario is that this won't work for you at all.

      Ha ha, I will absolutely take you up on that sometime soon!

      It's so true that there is an amazing lack of understanding about what this medication does and how it works.  But by word of mouth, we are getting things corrected - slowly but surely.

      I am always so thankful to this forum and website because it directly helps so many people.
