I'll start from the beginning. Of May 2015, I was experiencing sensitivity of my ear. noises in general wouldn't be loud but certain noises would hurt my ear. I would be playing my guitar, students talking in class, dog barking and my ear would hurt. Basically anything somewhat loud or high pitch( happening to my right ear). Sometimes my ear would hurt at random( both ears mostly right). My left ear would twitch non stop as well. It then stopped and I felt better. Of mid July is started again. My left ear started to become sensitive. Twitching stop, but the ear sensitivity was bothering me. Then I started to develop ringing and lot of ear pain in both ears. I went to the ENT twice but by the time I had the appointment the sensitivity went away. He Doctor said it was tmj, but I'm not sure about that. Ive never had jaw pain. The ringing went away for about 1 week but then came back, so it's been lasting for about 2 and a half months( the ringing). The ringing sound like when you turn on an old box tv. Finally, it's October and I still have the ringing and sometimes pain in my ears. Can anyone relate or help?

Many condition are associated with pain and ringing in ears. If you think that the sound may keep time with your heartbeat, it may keep pace with your breathing, it may be constant, or it may come and go then It may be tinnitus.
Recognised the mystery of your condition and get right treatment for the same.

Have you had your hearing checked? Is it normal? I suspect you have some mild level of hearing loss and nerve damage (maybe from loud music)? You don't seem old enough to be age-related, but you didn't state your age...just that you play guitar, so that's what I'm going by.
There's a phenomenon where the ear can't hear well at a certain decible level and then the sound is perceived as "too loud" or bothersome just a few decibles higher. An audiologist may be a good next appointment for you. S/he can refer you to another specialist if one is needed (ENT, Neurologist) and his/her diagnosis can help.
This is a longer shot, but I'll mention it. You could have a B12 deficiency. You'd have other problems if that were the case (you can google the symptoms), so it isn't the likely cause for you. But, for me, my ringing became far worse with low B12 and after injections/supplements it's so much better!! It was also overwhelming to be in any kind of noise, even if it wasn't actually "noisy"...like your experiences.
Good luck to you!!
