I’ve been losing my sanity over this condition, please help.

hello, I am a second year medical student, and I’ll try to get my symptoms across as clearly and as concise as possible, any help would be appreciated, thanks in advance.

Around 3 weeks ago, I was with my girlfriend, and we got a bit intimate, not intercourse per say but we ended up having oral sex along with “dry humping”, the oral sex was unprotected, but I didn’t really think twice about it because I had been intimate with her before. Little did I know she had a small cold sore (HSV-1) outbreak... One thing I should note was that the dry humping was very gigorous, and after the whole ordeal, I really felt sore on my penis. Another important factor at play here is that both my girlfriend and I are virgins. 

Two days later, I was having unreal dysuria (pain while urinating), the kind where you’re internally screaming everytime you have to go. And as I pulled back my foreskin (yes, I am not circumcized), I notice two ulcers on the underside of my penis, right behind the glans, i believe the area is called the frenulum, but correct me if I’m wrong. The ulcers were fairly painless, and it did not feel like they were oozing with anything, but I’ve had enough aphthous ulcers in my life to notice an ulcer when i see one, so this got me really alarmed. The tip of my penis was also a bit red and swollen, and when I looked in to my urethra, it didn’t look normal, looked like a sort of infection, more bacterial looking than viral. So the next day I go straight to a urologist, and upon inspection, he immediately told me it was not herpes, but didn’t really explain what it could be, just that it looks like a bacterial infection. Stubborn me kept on insisting it could be herpes, because to be honest, if you were only to deduce from patient history, herpes seems like a spot on diagnosis. He told that I had two genital warts and a bacterial infection and asked me to do a urinalysis, a blood test for possible STDs (virology), and a urethral swab for ghonorrhea and chlamydia. The urethral swab came back negative, the hepatitis B and C came back negative, AIDS came back negative, and my urinalysis came back with few pus cells, a slightly raised WBC count, and that was it. He told me to use ACYCLOVIR 5% ointment which got me more alarmed since he told me that it seems that i might have a genital wart, but it doesnt look like herpes, which didn’t really make much sense to me, and he also prescribed me LEVOFLAXACIN antibiotics 500mg per day. I took the meds and applied the cream, The following week it went for the worse at first, the few days after the doctor visit, my inguinal lymph nodes were swollen to around twice their size, I was having a slight fever, I could feel a sort of vague aching pain in my lower back, legs, the pain felt like it was a muscle ache, and surprisingly sometimes it would hurt on palpation, so the pain would consistently be somewhere for a while, but afterwards, travel to another location. Everything just felt swollen down there. But around 5 days after the doctor visit and with proper antibiotics and acyclovir treatment twice a day, the two ulcers/warts were completely gone, and my dysuria was almost gone. But I still felt a general feeling of uneasiness in my groin area. at this time I went to another doctor, who also examined me after I told him the whole story again, mind you the ulcer/wart was almost gone but still barely visible and the whole glans and urethra were still quite red. I explained to him what I had and after examination, he also told me that I just had an infection, and that herpes is very unlikely. I thought maybe because I got the ulcers/wart before the incubation period, I was starting to feel better, because I was spiraling down depression thinking I had herpes. After all this he told me to do a complete urine test, the one where they try to see any reactions against antibiotics as its cultured (also i think its worthy to mention that the urine was collected from the beginning of the stream). So i did the test, but what’s very scary is that it came back completely negative with no growth (at this point it was two to three weeks after the first doctor visit) because the cultures take time and I skipped some of the parts out.. At this time, I also went to do a hsv type 1 and 2 IgG and IgM blood test just for the sake of my sanity and both came back negative. I was also starting to feel some tenderness on my abdomen, mainly the right hypochondrium, and sometimes periumbilical, really it could appear anywhere, so I also did a Liver Function Test, everything was normal. The last two days I haven’t been able to take my antibiotics in time (Ive only been on two weeks of antibiotics) and before these past 2 days I was completely fine just a bit sore still but recovering, now i have a strange itching feeling in my penile urethra after ejaculation, and on palpation (squeezing) that doesnt seem to go away when it’s rubber. My anus seems to have an itching as well but that could just be from sweat since it seems that sweat exacerbates both the urethral and anal itching. Why am I starting to show new symptoms, before I didn’t have any sort of itching, so this is completely new. Id go straight to my doctor, but he’s currently not in town. So can anyone please help? Could my ulcers/warts be from trauma? Could this be urethritis or prostatitis caused by a bacterial infection? If so, how can my urine test come back with no reaction from any antibiotics?     If you had your urine tested shortly after your antibiotics course, there would be no growth due to antibiotics still in your system. Itch could be thrush or other opportunistic infection following antibiotics.

      Yes, I went to my doctor again today, and explained to him the itch on compression and palpation, he told me that this could be due to some dormant bacteria still in my paraurethral glands which can contract with ejaculations or any squeezing and exacerbate the situation. I’m almost completely healed now, thank you for the reply!
