There are people more experienced than me on this forum but this would depend on how much you are currently taking and where you have got to and how fast in the tapering process. You will need to take enough Pred to get the inflammation under control and only start tapering down again when you have become more 'stable'. It would help others respond if you supply a bit more detail I think.

Horse1018, sorry you are having a flare. I have had several flares, how much of increase, depends on how much you are currently taking and how severe the flare. But I do believe in acting quickly. Taking extra Pred before the inflammation gets ahead of you. The time I didn't I ended up going from 7 mg to 30 mg, so now I act very fast. Good luck, think positive! 🙂

The dose of Pred should be established by Dr. Other parameters like example GCA should be checked at the start. You must follow the Dr decisions since the only one person suppose to be responsibly for your treatment. Other patients suggestion should you reviewed but One Person One Decision should be followed.
Contact with the second Dr could be supporting but the results of the treatment you should follow. In case of failure of the treatment, in your opinion, change the Dr.

Changing doctors is not always an option. There may not be another doctor available. Short term is the Rheumy available on weekends. One is not in the area here. Still smiling! 🙂

It is impossible to make a suggestion (which is all any of us can do, your doctor is the boss) without far more info in terms of current dose and previous history. Why have you flared? Is it because you have over-reduced your dose, either going down in too big steps or too fast, or is it a real increase in the activity of the underlying autoimmune disorder while you were at a stable dose.
Many experts in managing PMR suggest adding 5mg to the dose you flared at - that will certainly help if the flare is due to overshooting the right dose and you haven't waited too long. If the flare is well established you may need more. If the activity of the disorder has increased it is difficult to know how much you need.
The same argument applies to the following reduction. If it was just that you slightly overshot the dose, you can usually raise the dose for a few days until the symptoms have resolved and then go back to the last dose at which you were OK, i.e. slightly above the dose where you flared. If the activity of the disease has increased - there is no fixed answer. You have to start your slow reduction again.

Very well stated EileenH, reducing from 12.5 to 11 mg 4 to 1 DSNS, failed twice go to 10. Not a bad flare, but heavy activity, just cannot slow down. Rheumy agreed, that's a first!
