Iliopsoas tenotomy results

I had an anterior total right hip replacement 7 months ago since then progressively gotten worse and worse with groin pain which has just ruined my life. Lost my company, boyfriend, etc. Im just gotten 2nd cortisone injection which i think is a waste of time. The ortho surgeon who did hip sugery at Cleveland clinic has been MIA since surgery... Ive had Mri which really showed the psoas intact. No useful info.

Another surgeon i am seeing wants to refer me to ortho arthroscopic dr for tenotomy..Has anybody here had that done?? What were results? I am desparate and want to get my life back. I so wish i had never had hip repkaced as i was better before.

P.s. the Drs assistant did say that since 2014 hip mfrs changed femoral head to make it bigger for stability that they have seen a lot of this

    I’m hearing your pain. I too had anterior hip replacement done June 2016,  ground pain, poor knee flexion & leg extension walking very painful. I too was sent for steroid injection got very marginal relief but pain came back. From the get go it felt like something was irritating my groin. I eventually got a second opinion & Lo & behold surgeon said cup protruding & rough edges rubbing on my psoas!  I agreed to have revision surgery & had a posterior approach 8 weeks ago.  Just this last week I’m walking unaided but still with groin discomfort. 

    Surgeon replaced the acetabular cup with a smaller one, I had to have a bone graft, screws & also a psoas was released. 

    I’m seriously hoping the groun pain eases off & that it’s going to be a slow recovery due to what had to be done.

    I had my fight hip replaced 6 years ago; posterior approach & was completely recovered at 6 weeks! 

    This left hip has been a nightmare. Maybe you’re onto something when you mention the size of the cup.

    I would get a second opinion if I were you as living in pain is so draining.

      Unbelieveable! i am sharing in your pain...my pain is constant...i am having problems in South florida finding a surgeon who does Hip Replacement Revisions and takes my insurance!!

      I just got another cortisone shot in the iliopsoas bursa, the side effects are horrible...my blood pressure is through the rough and no results!

      What i have found out is that in 2014 manufacturers started making the femoral head much bigger so cap had to be bigger which has started all the groin problems...Alot of physicians are still denying this...My dr who did the hip replacement is oblivious as he wants to take no blame

      Please keep my updated on your progress as i am right behind you...Have you lost any range of motion?

      I’m in Australia so I will check to see if same scenario as your femoral head & acetabular cup.  I will get details of my components & let you know how I go.

      So I’m nearly 8 weeks post revision & yesterday I did a 38 mins walk without my stick. Wasn’t easy & today I had to take a Tramadol to ease the pain. I can bend my knee up & down and there is a background pain in my groin.  I’m hoping that eases in time.

      I can bend down & touch my toes. Lateral steps to the side are very tricky & I need to practice these more. I’ve been so busy working that I’ve let my rehab slip 😔 

      I’m 51 and pretty fit & strong but this has literally changed my life & almost broken me😔

      I feel I am making small progress. Time will tell.

      The groin pain is life-changing. Cortisone shots have not helped from my research the only thing that has helped other people has been a tenotomy with a change of the femoral head otherwise it just keeps happening. Most orthopedic surgeons aren't admitting to making a mistake and putting too big of a femoral head in because that would mean legal implications. Please let's stay in touch with each other as I think this is been awfully unfair and as you can tell there are many of us now

    Whoa, sorry to hear about your serious pain.  I hope you find pain free resolution soon.  I'm going in for bilateral on Friday with both my fingers crossed.

    Very scary.  Won't bore you with my groin pain problems but the only thing that eased the pain was a muscle relaxant drug called Baclofen recommended by my physio as the muscles round my groin were going into spasms and making things even worse.  Might not work for you but thought I'd mention it.

      Thank you Carol. I will mention it to my primary. I don't think they groin problems will go away unless we have a tendon release and they change the femoral head because unless you remove the cause it cant change! What a mess!!! Is your ortho surgeon helping you?

      I'm going to need a revision if future as my op. wasn't successful but as I use a wheelchair because of MS  the  revision is very difficult so I've decided to try and cope with the groin pain for as long as I can.  You're probably right about the Baclofen but it did help me.

    Hi

    im only three weeks post op but I have very noticeable groin pain that isn't getting better in spite of everything else seeming to gradually improve. Im ok when at rest but any kind of activity brings on significant pain. 

    Groin pain seems to be a really common problem. I had the SuperPath approach which is minimally invasive and thought the groin pain would be gone as prior to the operation it was the groin pain and locking that prompted me to have it done. The locking has gone but groin is very painful. 

    What concerns me is groin pain after THR seems a new issue that seems quite common.  My mother had two hip replacements almost 20 years ago and never had groin pain so it is concerning so many of us seem to get the problem. 

    I go for physio tomorrow which is my first time and will mention this as I don't think exercise is the way to go yet physios seem to think the approach of no pain no gain applies!!

      Physio is not an orthoo surgeon..if you will do research exercise makes it worse, at least from what i have read. May i recommend you go back to your surgeon and/or get 2nd opinion from an ortho surgeon...

      A lot of people are having this problem and its not the norm...i believe its because manufacturers have made the femoral head bigger in recent years and it has created this. .most surgeons i am finding out wont admit to a too large femoral head for fear of lawsuits.

      I have tried everything and nothing has worked..the daily groin pain has ruined my life...tenotomy is next step..get very real with your ortho...good luck!

      Hi Gary

      In my opinion exercise & physio are not the solution. I wasted a year going down this path & the other favourite that gets dished  out ; steroid injection.

      What I think is the issue is an issue with the acetabular cup and/or femoral head. It’s a mechanical problem irritating the psoas. 

      I had my right hip replaced 6 years ago, posterior approach & was pain free & exercising after 4 weeks. 

      This left hip was replaced last year; anterior approach & within 6 weeks I could tell it wasn’t right. 

      I’m going to do my research & ask for comparison in sizes of the hardware used in my 3 surgeries; right hip 6 years ago, left hip 15 months ago & revision 2 months ago.

      I’ll jeep you posted.

      its interesting. I haven't seen with the old posterior method any mention of groin problems, it does seem to be the so called  quicker recovery methods that incur the groin issues. Your case does seem to highlight this differential somewhat.

      I think you are 100% correct. Mine was anterior approach and its been horrific groin problems for 8 months! I am searching for a dr to do tendonitomy and femoral head revision
