Inside Ear twitches, thumping

So ive been having some pain when i swallow on the right side of my neck right under my jaw all the way up to the year. Ive had it for a few weeks. It has got a little better but at night the pain gets worse. I went to the ER a few weeks ago bc it was hurting so bad, they said i had no infection and sent me on my way, they tested for strep and other stuff which was negative. I woke up very early this morning with a weird twitch inside my ear. A thumping sound that i can feel and hear. Ive been having bodywide twitches since december especially in my right foot. Doctor doesnt know what it is. i suspected ALS could be possible cause but doctor wont refer me to a neurologist. Anyway, the ear twitch is completely new symptom. Somethings gotta be going on for me to have pain in my ear and my throat. has anybody ever had this issue???
