Intense chest and back pain when laying down.

I'm a 14 year old male, and I've recently have been spending a lot of time in bed. Every time I lay down, If I change my position even a little bit, I get pretty intense pain on the left side of both my chest and my back. The pain feels skeletal on the chest, and muscular on the back. When I sneeze or cough when laying down, it almost feels like a gunshot, with intense pain for 45 seconds, and lingering pain for 5 minutes afterwards.

A few weeks ago, I fell and hit my head, causing a moderate concussion. I'm pretty sure these two events aren't connected, however the pain did arrive around the same time.

I really don't know what to do. It gets worse by the day, and I'm really concerned.

Edit: I haven't stood long enough for this to happen, but it also flares up when I stand up nonstop for longer than 15 minutes.
