Intermittent Testicular pain

Hello, wondering if anyone here can help me narrow down my issue or have had something similar happen to them. I am 18, and a fairly active guy. I have had intermittent testicular pain starting about 4 years ago if I remember correctly. When I say intermittent, I usually have this pain once or twice a month. It is very sudden, VERY sharp pain that occurs mostly when I stand up from sitting or make an awkward movement. This pain seems to randomly be on the left or right lower part of my testicle and shoots up into my lower abdomen. When urinating when experiencing this pain I also feel a very sharp similar pain causing me to have to stop peeing and take breaks because the pain is so unbearable. I have visited the doctors multiple times over the years and have checked almost everything off the list, no kidney stones, nothing wrong with my urine, no lumps on my testicles, no testicular cancer. I've had multiple ultrasounds, a prostate exam. and a Ct scan and everything has come back negative. Due to the pain being so sharp my doctor was concerned it was "intermittent testicular torsion" and so after being seen by a specialist I was operated on just over 4 months ago for testicular torsion repair. My testes were sewed in place and I believe my appendixes on both were cut off or put in place or what not. Since then I have had multiple instances where I have had very sharp pain just like before the surgery. Before I had surgery I was on a semi regular prescription of MiraLax. I have been constipated before as a younger kid and for the past few years I can notice a substantial amount of blood pretty regularly on my toilet paper while wiping. I don't know if these things are related in anyway or if hemorrhoids can cause this pain but I don't want to leave anything out of the question. Also if I'm going to add it to the list I have had multiple hernia surgeries in the past but my doctor said that they wouldn't be the cause of the pain. Lastly, I have had a couple instances while masturbating that I have had this sharp pain, but this has been an extremely small amount compared to the amount of times I've had the pain in total. I have scrolled through the forums briefly here and am simply lost on what my issue could be. Could a pain like this be coming from constipation? Or hemorrhoids? Please I need any help or information I can as I am sick of dealing with this pain. Once again this pain happens for the most part randomly and seems to switch from left test to right test. I am out of options at this point. Thank you.

    I was gonna definitely suggest a hernia, but since you said you've had multiple hernia operations already, we can rule that out. I would suggest finding another urologist who can get to the root of your problem. I definitely know what that "come and go" pain feels like, but mine was indeed a hernia. I don't think hemorrhoids can cause that type of pain, but I'm not sure. I say that because I have a small one back there, and have no pain from it whatsoever. But you surely need another urologist to check you out, especially if you're getting any type of pain during masturbation...that's really odd. Plus, you don't want a pleasurable experience to continue to be painful. Good luck!

    Have you been told that you may be experiencing retacting or asending testicles. Prior to 1993, my testes were retracting back into my inguinal canal. It created a sharp crushing pain of my testicles. Then on April 29, 1993, at age 42, I had them tied down with an operation called orchiopexy. But it should have stopped the pain. It didnt. They can't enter my inguinal canal anymore, but sometimes I still feel the crushing sensation. But I am not like you since my testes are atrophied and are smaller than a hazel nut sized shape. I am a xxy male, so I am sterile. If I don't get hormone injections, my hormone level is 34. Below normal.level.

    If you have normal sized ones, I don't have any idea why you are having pain, since all of your tests came back normal.

    But has a doctor checked your hormone levels.

    Two months ago, I forgot to get my hormone injection and I had horrible crushing pains in my testicles again. But after I got my hormone shot, less pain but they are tender.
