Irritated penis head & weird spots

Hi all,

Bit nervous as first post here but i am worried and need advice. I've been getting a lot of redness and irritation with my penis for probably a month on and off.

i woke up today with these white splotches on my head. Does anyone know what this could be?

I do have herpes so it could be an outbreak but I haven't had one in a long time and it doesn't usually look like this.
