Is this Balanitis? I'm 21, had it most of my whole life, no treatment has worked

Hello, I've seen a doctor many times about a mild condition I have on my penis glans, have been prescribed 3 different creams including Betnovate and a Hydrocortisone, a course of antibiotics and a swab which said I was clear and still the condition remains. I used to have a tight foreskin up until last year which is now sorted from stretching, and I think the condition began during my teenage years when I wasn't able to wash the glans. It often feels dry and can sometmes feel slightly painful when the foreskin retracts, almost like its rubbing and the sensitivity everywhere is quite high still, not sure if this is due to the condition. The urethra opening is always red and inflamed and red areas appear frequently around the glans itself. Any advise on what I should do would be much appreciated, have attached a photo too. Many thanks!

      @maxpower2020 here, i dont believe its either of those. this all came up just days after risky oral sex. its either that or self inflicted injury due to anxiety and frequently checking my urethra. my only visible symptoms are urethral. thanks for the reply

      All of us seem to have caught this out of different routes (some after unprotected sex, some of them after protected sex and in my case it started after masturbation).. So with all your cultures and STI tests returning negative and taking my case into consideration where I've not engaged in any sort of sexual relationships I'm much convinced this is not an infection but as a result of an injury or trauma to penis. I guess it would take time for penis to heal but I think how long would it take still depends on the injury. With my foreskin and Glans very dry I'm also thinking about undergoing a full circumcision after the pandemic settles down in my country.

      yeah my symptoms get better after a couple weeks but masterbating or having sex, even with a condom make it worse. hope to god this doesnt last the rest of my life

      Yours is defiantly something different. Might just be skin irritation and needs to heal. Urine doesn't help with the healing process either. So maybe every time you urinate, wash the penis in a cup of water so there isn't any residual urine. Also, set a urination schedule? For example drink at certain times of day so you void 2 or 3 times per day, and the rest of the time, the urethra has a chance to heal. Additionally stop checking so often, as constantly spreading the meatus open further irritates it. And lastly as I suggested above, trying soaking in salt water, opening up the urethra so a bit of the water goes in, as this promotes wound healing and kills pathogens if they are present.

      why do you say urine doesnt help healing process? one theory i have is that my body isnt creating a response to an unknown bacteria so i dont have cloudy urine or discharge. im having troubles finding a clinic that can and will do EQUC testing of my urine, to find bacteria that isnt discoverable in a standard culture.

      Urine is your body's waste product. Can be acidic in some cases and contains urea which is a product of ammonia. So it's definitely not something you want on your skin long term (especially if the skin is already damaged/compromised). Conditions like lichen sclerosus are often contributed from urine in the area.


      Which particular bacteria are you hoping to find? Your body contains trillions of bacteria. Every time you touch a table or door handle you come in contact with millions more. Every time you eat anything diary, you just ingested millions more. etc.

      so other than wiping dry, do you know of any other way to help the healing process if this is just a minor injury? to be honest i actually am hoping to find any bacteria that could be responsible for urinary tract infections, mostly just because its potentially an answer to my problems. like i mentioned before, my issues didnt start till oral sex exposure with a risky (highly sexually active) partner. so im guessing it could be a bacteria that is common in the mouth, or asymptomatic in the mouth.

      Doctors recommend shaking and wiping any excess urine that remains. And as I mentioned before, soaking in saline might be beneficial. Well if you don't have any luck finding bacteria, you could just try taking certain antibiotics prophetically and see if there's any improvement.

      also, why am i reading that acidic urine is good for preventing new growth of new bacteria?

      my latest doctor wont give antibiotic if she cant see proof infection. my urologist gave me doxy no questions asked because of my symptoms and exposure. the doxycycline has antiinflammitory properties so it helped with urinating. i thought it was relieving me but was probably jusy helping woth swelling

      i took the "mAicroAgendx" test and group b strep UTI came back. i also sent urine to an infectious disease specialist at university of michigan, which came back with the same results, AFTER, a urinalysis and urine culture came back with no results prior. my doctor said that is because the other tests are not as sensative, i had low bacterial load count in my urine but im getting amoxicillan to treat it now.

      The problem with those ultra sensitive tests is that they detect error contaminants. But maybe your case is different. Best of luck!

      yeah in the semen test i had Corynebacterium tuberculostearicum, Streptococcus agalactiae, Staphylococcus epidermidis. all lower than 100,000 DNA copies per mL which is usually "fine". my doctor said UTI is usually treated if its over 100,000 or if theres symptoms. my urine had strep agalactiae only. it literally only hurts after having sex or masterbating, so my guess is its either my semen that is bothering my urethra or its an injury from when i was checking my urethra too much a few months back when this was stressing me out a lot more
