What you may have is a combination of different things going on here: first I would ask do you have skin problems or issues anywhere else on your body?
As well as, what you need to think about is everything that is in contact with the skin on the head of your penis?
For most men this would be soap, shower gel, other forms of body wash, stuff applied for or during sex and masturbation - and so on.
You need to know that the skin on the head of the penis is one of the most sensitive of the human body, it reacts to contact with things as simple as soap and particular shower gels when the rest of the skin on the body simply does not.
When there is also the possibility of a skin problem, such as psoriasis or eczema the genitals is one of the areas where these issues can show first.
What is in contact with your penis head in terms of soap, body was, actual Lubes, things used for sex and / or for masturbation?
Also, if you shower regularly, do you have the head of your penis exposed throughout the shower, or only when you are cleaning the head and foreskin? If it is the later, do you do this at the start of your shower, no particular time during, or at the end of the shower ..... ?

heya, i do not have any other skin issues anywhere else on my body, could exczma just affect my penis glans? i dont wash the head of my penis with anything other than water, and i make sure i do so last in the shower and then make sure i dry it straight after, i do not generally use any lube or similar things.

So yes, because of the nature of genital skin issues that could potentially show anywhere on the body can initially or only appear anywhere on the genitals.
I would be surprised if there is not an element of something that is in contact with the head of your penis that is enabling, or even causing the issue to show as it is.
I think the best course of action would be to see a Dermatologist who may try you with several different medications until this clears up, or at least provides you with a diagnosis of what is going on
