Itchy Anus. Piles or Chronic Fissure? (With photo) Please help!!

Hello,

I'm 19 years old, and I have been suffering from anal itching for a year now and it is ruining my life. I have noticed that there is a slight lump or 'skin tag' around my anus, which led me to be confused if it is Piles or a chronic Anal fissure.

I experience minimal to no pain whilst passing a bowel movement. (It is very rare but can happen). The only thing bothering me is the constant itching. ( I am a clean person by the way.)

Ointments such as Calmoseptine don't seem to help with the itching.

I have attached a photo and it would be much appreciated if you could help me figure out what it is.
