Started on 20mg  prednisone  of 4 days ago

Symptoms started to improve

Yesterday I felt very unwell and on call Doctor told me to reduce to 10 mg  still feeling sick, shaky and racing heart and pain back

Do you have to take all prednisone in one go or can you spread it

Does anyone else have these side effects

Hi there I was diagnosed with polymyalgia in July started off with 15mg a day was told to take them with a good meal so I took them about 5pm it was almost immediate improvement from the polymyalgia which had rendered my hardly able to walk and couldn't pick up my right arm above the elbow but then came the side effects noticed me hands were shaking not bad but definitely shaking then every time I sat down and then got back up I felt dizzy and had to hold onto things till it passed and then the insomnia which is awful also heart racing and palpations posted on this forum and lots of people had the same symptoms I have now reduced to 10mg a day the dizziness has gone and the shaking heart races occasionally and still a few palpations but better still cannot sleep although when I go to bed I am really tired I might had though that I have never been a good sleeper hate the predisilone but the thought of going back to the polymyalgia is terrifying

Try taking the pred at breakfast, will have worked by the time you're going to bed and you'll sleep better! 5 o'clock is quite late to take it! Hope it helps!

Stress won't help you.  Have you been doing a bit of rushing around while you get ready for your holiday?  That could easily slow down recovery.  That being said, I am concerned that you are still experiencing significant discomfort after two weeks on pred.  Are you and your doctor sure it is only PMR involved?  If so, you may be one of the people who needs a slightly bigger dose at the beginning to get the  inflammation properly under control.  You may also be one of those who do better with splitting the dose, so that, for example, two thirds is taken at breakfast time and the other third perhaps with supper.  Some of us metabolise pred faster than others.

Why don't you check with your doctor if you could try a higher dose, perhaps even 20 mg, for a few days?  That could do the trick.  And if it doesn't it might warrant further investigation.  I'm sure there will be others along who can offer some expert advice.

good suggestion Anhaga. I was put on 50mg as a starting dose and dropped dramatically over the first month - hit it hard and fast was the theory - it worked a treat!

I was one of the lucky ones whose pain resolved completely within two or three days with 15 mg where I stayed for a month.  I even suspect I might have been okay at a slightly lower dose, but did get down to 10 in about two months.  Much slower since then!

A diagnosis for Polymylgia is generally measured by the success of starting on prednisone as symptoms generally abate very quickly after starting it. Two weeks does seem like a long time to get very little reaction so maybe a visit back with your doctor might help, either, as suggested, to increase the dose or investigate something else. On the side effects from pred, it can make you feel very unwell as you get used to them, so definitely time is needed to adjust to them. A holiday might do you some good in that you'll be forced to rest and take things easy so don't dismiss it too soon. Hope you'll be feeling better very soon!

Hi this there you definitely need to see your gp the predisilone if you are taking the correct dose you work in 24hrs I was perscribed 15mg daily and  at that time could hardly walk couldn't pick up my right arm above elbow and the left wasn't much better it was a nitemare but within 20hrs I had a 90%reduction it was like a miracle perhaps 15mg isn't enough keep us informed

Good luck at the Drs Robo.

I started on 20 mg at Christmas time and felt as if I'd got my body back. I still had a few problems with my shoulders so th rheumy put me up to 25mg. I have slowly reduced have now just got to 10mg (split 50/50 am/pm) and will stay here for 6 or so weeks.

Everyone has a different reaction to PMR firstly and then to the pred so read as much here as you can and with your Dr you will personalise your "recovery". Just remember that it will take as long as it takes so worrying or stressing about it will do no good and may well make things worse.

Have you been checked to find out if anything else is going on to cause the pain?  I read an article recently which talked about other things which can cause PMR pain, sorry I can't remember any more details, but at the time I wondered how many people might be going around with incorrect or incomplete diagnosis because classic PMR had been settled on as the only cause for the pain.  These are people like yourself who have not responded very well to the moderate dose of pred which acts miraculously on most of us.

Your inflammation levels don't really matter - what matters is how you feel. If you are still in pain then you need to keep taking the pred until the pain has gone. When it's subsided, then you can start thinking about a SSSSLOOOOOWWW reduction.

There are a number of other ailments which mimic standard PMR and you should be invetigated for these.  Not a medic, but I did find an article about this on the internet.  You could look for polymyalgia differential diagnosis.
