Left testicle pain. No varicocele or epididymitis.

Hello. I'm 20, and I've been struggling with testicular pain for a week now.

My left testicle is painful and tender to the touch. The pain radiates to my left lower abdomen, my left leg, and a couple of times the left side of my penis (where I had a small tear after a rough session a few months ago that left a painful scar). I've had an ultrasound done today, and nothing is wrong with my kidneys, bladder, and testicles so far. The pain seems to be at its worse during the evening. When I'm distracted enough during the day and when I masturbate, it's gone. Nothing has changed with painkillers. I'm thinking maybe that tear/scar (that looks like a tiny keloid) under the head of my penis is the reason, somehow, but it doesn't hurt unless I touch it.

I'm going crazy. At this point, I just want to find a problem and get the pain to stop. Please help.

    Hey...

    If you are relieved after masturbating it might be an infection or inflammation. Epidisymitis is not the only kind of infection one can have. Many STDs can cause sth like that. Get checked for chlamydia, or Candida for example. btw do you feel that your testicles are burning and is your semen yellow-ish after ejaculation? then this might be an answer.

    i really don't know whether the scar has sth to do with it but i wouldn't connect the case with it theoritically at least.

    the only thing that doesn't match up is that the problem is on your one testicle and not both. i would run some urine, semen and blood tests to exclude all infectious factors tho.

      I'm sure I don't have any STDs.

      I got a urine test today. Getting the result tomorrow or the day after. I'm hoping it's a small treatable infection. If the urine test is clean, I'm going to see a urologist.

      I hate being in the dark. I'm extremely paranoid, close to hysteric... The pain can be very uncomfortable and I can't focus on my books or on enjoying anything really!

      I tried stretching my legs, and I think ibuprofen relaxed my leg and abdomen for a short while. Or it might be this mysterious pain playing tricks on me.

      Glad that scar is probably unrelated. I'll check a dermatologist one day if it gets too wild.

      Hey dude relax, it is probably nothing. you def need to see a urologist anyway! but don't worry most probably it's sth minor. wish you all the best, keep us updated.

    So apparently, the first doctor was wrong! Hence, wrong title 😦

    My urine test was clean. I had nothing. So I went to a specialist today. The urologist did a physical exam, then somehow found the source of the pain: my epididymis! It was so painful when he pressed on it, I was close to shouting. It's still throbbing and it's been a couple hours.

    He checked my kidneys with an ultrasound, I had no kidney stones. I did those exercises where he told me to turn around and cough. He checked for reflexes or something? I'm still confused about that. He kept telling me to cough harder, and then he told me I did have an epididymitis. I don't know what caused it, but he seemed pretty sure. He gave me Ciprofloxacin, 500mg twice a day for 14 days, and told me to go back for a check-up in 7 days. He explained that just painkillers wouldn't work, and pain should go away after 48 days with the antibiotic.

    Hoping to feel better by tomorrow, and I already prepared a few questions for him next week ><

      Quick update!

      It hasn't been 48 hours yet, but I noticed that the pain has dulled (thankfully!!) and I can feel it just around the epididymis. Stabs of pain in my testicle (sometimes down my leg) can get sharper from time to time, but it's bearable compared to before. However, on the outside, my epididymis has become very tender and a little swollen. I've taken Cipro 3 times so far, and i don't know if that's a good or a bad sign.

      All I care about is that the pain feels better now! I'm going on a small family trip (bad timinggg) so I'm hoping for the best T_T

      Good, you may take ibuprofen or Naproxin with the consultation of Doctor.It will help to reduce inflammation. Enjoy your trip.

      Update!

      I'm back home, and I'm glad to say I'm feeling much better every day. The pain worsens under AC or when I sit down for too long, but it's been a pleasant few days compared to the week prior to my visit to the doc.

      I didn't take any ibuprofen, and for now I still have a dull ache or the sharp pain down the left leg from time to time. I'll ask my doc if I can take painkillers, I assume that would make me totally pain-free

      I didn't have sex or masturbate since starting Cipro, and it turns out I'm not a fan of abstinence T_T

    So I guess I should make a quick update.

    The pain is back. It comes and goes, but it's less of a discomfort and more of a sharp pain again.

    My treatment ends tomorrow, and I can't get a hold of my urologist - he's on break and I didn't go for my check-up last week because I got a cold and then had to take a Covid test. Turns out he doesn't work this week. Anyways.

    I read that discomfort can last for weeks AFTER taking antibiotics, but for the past 2 days it seemed like it got worse somehow. I don't know how. I thought it was getting better, but the pain is still persistent.

    It was dull for the past 12 days since I started the treatment, but it seems like it got worse now. Maybe I got the wrong antibiotics...? I don't know. But I'll keep trying to call the doc.
