Long Term Burning Sensation

Hey everyone!

I have no where else to go and I am hoping someone can help me out here. I am a 25-year-old male, with a height of 175cm and weight of about 80kgs. I have ideal overall physical health, I have optimal blood pressure and heart health. I am physically fit, I have no known other physical co-morbidities, I do have a history of anxiety, particularly around health issues.

Over the past 2-3 years, I have had this strange on-and-off burning sensation in my penis, it's directly at the tip of my penis and is ruining my life. It is constant, medication such as analgesia or anti-inflammatories don't make a note worthy difference. It has been on and off for sometime but has returned over the past 3 or so weeks. This sensation can sometimes come on when I am sick with a cold or flu.

It's a burning / stinging pain, all of my tests have come back negative, I had rectal chlamydia 3 years ago and was treated for another rectal infection (mycoplasma -genitalium). I have never had a penis issue prior to this, apart from a period of over-active bladder syndrome 2 years ago that resolved after a 6 month course of Betmiga.

Peeing can occasionally make it worse, depending on the acidity of my urine. So does vigorous sexual activity, spicy food and any other usual bladder irritants.

I have an average diet, I don't consume a lot of bladder irritants because of my previous overactive bladder syndrome. I drink one coffee a day and 1-2 soft drinks, the rest is normally a decent amount of water. My diet is probably high in sodium than the average person, I crave salt more than sweet. I would consider it well balanced apart from this.

I exercise frequently, I take no regular medication at this stage but have recently finished a course of anti-depressants (Sertraline).

My doctor and Urologist are unsure what could be causing this. The pain is constant now, I originally considered it might be an injury from masterbating too much but I have taken an extended break from this and it has made very little difference.

There is no evidence of contact dermatitis, I have also changed soaps, detergents, etc to no relief.

I am reaching the stage where I am willing to try anything, any suggestions or questions are welcome!
