Looking at having a 2nd surgery (pic included)

Hi all! I guess I'm looking for anyone with a similar experience. 5 weeks ago, I had a sphincterotomy. I went for my 2nd checkup today. For the last 2 weeks I have used the nifedipine/lidocaine and psyllium fiber as he directed after my first checkup 3 weeks post surgery. I put the N/L mixture both around and inside my anus 3x a day, sometimes more when spasms occur. today he said give it 3 more weeks and if still sufferingfrom the fissure, he would do a fissure surgery. Is this the same surgery as I just had? I didn't think to ask him as my mind was going in all sorts of directions. has anyone else had this occur? Below is hopefully an uploaded picture of my anus with the giant skin tagsthat have developed after my first surgery (they weren't that big before!) that surgery was the most painful experience ever and I don't want to experience it again! I just don't know what to do. thanks for listening. image

clarification edit: I know the first Surgery I had, the sphincterotomy, Was a surgery to treat the anal fissures. But today he didn't call it by that he just said a Fissure surgery. I guess I'm wondering is a sphincterotomy the only surgery they use to treat fissures or is there a different one? and again, wondering if anyone else on here has had to have multiple surgeries to treat anal fissures? thanks again.

My god when I looked at the image I was horrified what have they done to you? I had fissure surgery back in 2004 mine were very bad, suffered for years with the terrible pain. After my surgery, I felt so much better and being still numb I expect the first B/M to be bad. It was not, I had been given very positive instructions by my Dr which helped me to get over it fast.
I have to be careful still, but I no longer suffer as I did before. The best advice I would give anyone who goes through this, is diet. Diet is the most important way to go. I no longer eat bread but do make sure I get enough fiber daily. My advice is trial and error what to eat till you find what helps and makes you comfortable.

yeah, I was horrified too! I call it my back door's door! lol. but yeah, I suffer from chronic diarrhea and soft stool, so he had me start taking psyllium fiber which has helped it firm up some, but not to the degree he would like. I'm working with a nutritionalist now to get my diet the way I need it to be that I can stick with, I meet with her Saturday. I just dont understand how the surgery didn't work the first time. I've followed the doctor's instructions to a T! But everyone's body is different, guess my butthole wants to be a butthole! Thanks for your support, just gonna take it one day at a time.

either these skin tags soft or hard ?if its hard its anusitis. have u had ur second surgery ? wt did they do now removed skin tags ?
