Can you tell me how long you have been suffering with this problem? Did these symptoms pre-date your tests at school, or did they begin as the time for the tests drew nearer?

Without rushing to judgement, that would seem to signpost the source of your unpleasant symptoms to a growing sense of anticipation and possibly mild fear, and this could very easily have created a state of anxiety in you that would produce the symptoms you have described.
The inability to concentrate is clearly an anxietal condition. The more physical manifestations you have described, difficulty in breathing or shortness of breath, plus the shaking, are common symptoms of a mild to moderate panic attack. It's a very unpleasant feeling Eddie, but if indeed this is stress related, you don't need to add to it by worrying there is something physically wrong with you.
I'm sorry your teacher doesn't understand, but I wouldn't worry about that. Are you part of a supportive family? Have you been able to talk to your parents, or siblings (if you have them)?
Apart from the obvious stress of exams and tests, are you dealing with any other problems that you are bottling up. Please don't write anything of a personal nature here. I am just wondering if anything else may be contributing?
Don't fret over this. Whatever is happening has a solution. You (with help) just need to find the root of the problem. You will do young man.

Well I'm sorry if my comments were not as supportive as I would have wanted them to be. My primary goal was to strengthen your understanding of the anxiety and hopefully another voice would assist you in normalising the physical effects so that you would worry less about them actually being 'real' or harmful.
I wish you only the very best and sincerely hope you begin to make an improvement soon.
Finally, for the sake of my own reputation on this site, and for the benefit of anyone reading your reply to me without reading my message to you first, I place on record my absolute lack of any prompting or suggestion to reveal underlying issues to me or anyone else other than a Health Professional, and most certainly not on here.
As is evident from the following quotation directly from the message you replied to and is posted for all to see;
"Please don't write anything of a personal nature here".
My very best wishes to you.
