My penis head/tip is painfully sensitive before orgasms

The tip of my penis is painfully sensitive before orgasms. It is not overly sensitive unless I am erect. I would describe the pain as about the exact same feeling as the refractory period from after an orgasm, only I experience this before orgasm as well. This makes penetration during sex impossible since any time the head/tip is contacted while I am erect, it is painfully sensitive. I am 99.9% sure it is not an STD since I have only had one sexual encounter during which I experienced this issue (I wore a condom). The problem also existed before the encounter. I have tried simple lotions and ointments (such as aloe or Vaseline) as well as an over the counter ointment to no avail.

I think if you went to see a Urologist they may simply ask you questions like this. The more diligent ones would want to make sure blood flow in the penis was working as it should. Alternative medicine practices would check for blood flow and make sure the nervous system of the penis is functioning as it should. Chinese medicine, for example may use Acupressure or Acupuncture to apply healing to another area of the body to implement healthful change in your penis.
Using this approach maybe you could consider how your posture or routine tasks your are doing could actually be causing a problem. So this could be, you sit at a desk all day, perhaps regularly with your legs crossed. Perhaps you do regular gym workouts where you are sitting and doing something strenuous at the same time? It could even have been a one-off thing that happened for you, or as I initially suggested, related to a health issue.
So yes, you have tried a few things - what I would suggest you try is to get yourself either some massage oil, or simply some coconut oil used for body skin and try creating healing massage for yourself in which you learn how to use gentle massage on your penis. The best time to do this is in the evening and you have taken a warm shower. Simple find how to use gentle massage on your penis. And yes, this is completely different from the pressure and actions during masturbation, so the idea is not to get an erection. If you do become erect simply relax and let it fade away, before continuing the massage.
What a lot of men expect is to implement something regarding the penis and they should get almost instant results if the process they have implemented is going to give some benefit. Because of this many men have done the right thing but have not given it long enough.
I would suggest you try the massage 2 or 3 times a week and you may need to do this for up to 6+ months before you feel any real benefit. The alternatives are go see a Urologist, or go to an alternative medicine practitioner ......

I only have one question - do you think it is okay to continue masturbating, or should I stop until my tip is feeling better?

Obviously, there are 2 approaches to this: continue anyway irrespective of the sensations you experience. Frequency of masturbation is an issue for many males. So if it is frequent for you at the moment certainly reducing to perhaps twice a week may help. The other approach is complete abstinence, giving your penis a 'masturbation holiday' could be part of the process of 'cure' - but a little treat, or light relief once in a while should be alright.
The most important thing for you to discover is how to effectively use massage and develop your technique to do this effectively. Posture for this is really important, so you need to find a location where you can do the massage - I use the edge of the bed with feet resting on the floor. If you need to use a bathroom location, pop the loo seat down and put a towel on it - but wherever you practice and perfect your massage feeling comfortable about it helps a great deal. This is why a warm shower can be really helpful and it relaxes the entire penis to enable better massage manipulation.
Start easy and gently, then when you have mastered a technique that works for you build on that. As I say, this is unlikely to be a quick process, best to perfect a technique first for what could be many months ahead .....
