My PMR returned from 2005

MY PMR returned from 2005 and my legs and hips are killing me i had the test for GC and am fine thank goodness DR has me on 5 mg of pred twice a day there are some days i can hardly walk i am really trying to follow the anti inflamatory diet any sugestions welcom

    Hi Gerio: I think you need more prednisone, like 20mg daily. I do not think 10mg. a day is enough to start. Try that. I had PMR for 2 1/2 years, have been without it for about a year now. I started on a higher dose and all of the pain subsided and I could function normally. Good luck to you!

    I started on 15 mgs and I am a small person, 105 lbs. You are not taking enough pred . Most people take once a day in the morning.

      I'm confused, Gerilo, are you saying that after almost 20 years at 0 pred, the symptoms suddenly came back? This is the first time I've heard of this happening. I'm really sorry and hope you can get yr symptoms under control quickly and with as little pred as possible. May I ask: were you challenged by any significant increase in stress recently?

      yes i have been free of PMR SINCE 2005 not enough flares to even notice or go to the DR Then I Couldnt walk one day and was at my neurolgist and she suggest I see my rhumy that the PMR had returned There were a few times I felt the PMR but it didnt last enought i thought i over did it for the day

      NOW I can really tell its back and yess stress has done it i try and watch the anti inflamatory diet thats what i did the last time

      They tested me for lupus but no thankgoodness this morning is much better but a headache i can walk a little better

      yess stree is a big factor

    My mother had PMR , got over it was in remission for several years, and then got it back again, and then went into permanent remission.

    I got to zero pred for about 9 mos and then it came back and has never left. I'm rarely successful getting below 15 mg for very long. BUT I manage my pred always trying to cut back very slowly and I live an ACTIVE, sporty , full life , mostly as pain free as any of my athletic friends in their late 60s.

    I agree with the others. A higher start dose of pred is probably needed. Good luck!

    What test for GC?

    5mg twice daily is a strange dose - in PMR the pred dose should be taken 1 daily all at once preferably in the morning. But the starting dose needs to be enough to combat the symptoms and that almost always means starting at 15mg if not 20mg. Then you taper the dose down slowly to find the lowest dose that gives the same result as the starting dose did - it is your guideline and why you need to start at the right level.

    I don't think your doctor really knows what they are doing and you might be better looking for a more knowledgeable one.

    Hi Gerio, I think a good starting point is either 15 or 20 first thing in the morning to try and get the inflammation under control. Once the inflammation is undor control and you are PMR pain free then start a taper regiment using DSNS and not reducing by greater than 10%. Good luck on the rest of your PMR journey, try to stay active, positive with a smile, I believe it helps. 🙂

      Thank you I agree to stay positive I try to live my dream everyday as like most i have many other problems with pain I never dreamed my PMR would come back like this I am so thankful for understanding family and friends

    *you need a much higher dose than 10 mg per day during a flare. I had to go up to 30 mg. my ESR was 99. I am down to 8.mg.

      not necessarily. It depends where you are in the first place. I was down to 1.5 and never had to go higher than 7 for a very short period of time to control the flare. A standard recommendation is to take 5 mg more than you were taking when the flare started.

      It depends - if the flare is due to overshooting the right dose in a reduction and you react quickly then adding 5mg for a week and returning to the old dose that you know was enough will often be enough. If that happened at 5mg or lower then 10mg should be plenty.

      If it is a resurgence of the disease activity you may well need to start again - but it would be unusual to need 30mg, that is high as a starting dose for PMR.
